Question: Hey doctor for about a year and half now I have had this burning sensation when I urinate. It’s especially worse when the urine is concentrated like when I first wake up in the morning or if I don’t drink any water throughout the day. I’ve noticed the burning is at the beginning of the stream not so much in the urethra and it’s more like a stinging pain. I have visited my GP about it and I have done several urine samples and also swaps of my penis which show no inflammation or infection at all and to add I got this before I even ever had sexual intercourse with anyone so this was prior to that. I have also visited a urologist and he said it was nothing major and not to worry about it however I am just concerned they’ve missed something. What sort of conditions can cause this burning pain that I’ve described can it be any type of cancer or is that unlikely and is there any other tests I can do like an ultrasound? I have noticed this problem in particular hasn’t gotten worse at all it’s just stayed consistent over the year and sometimes I don’t even notice it if I drink a lot of water that’s the main thing that helps also masterbating or ejaculating more so seems to irritate it and make the urine burn as well I’ve noticed but I’m unsure if that’s normal?

Do you have any other symptoms for blood in urine, penile discharge/ ulcer, abdominal pain, fever, decreased appetite/weight?

In that case, you may require ultrasound / CT scan of the abdomen, discharge culture sensitivity ( if discharge present) after urologist consultation as per evaluation to rule out any other possibilities for renal/ bladder stone, sexually transmitted infections.

Till then, keep your self well hydrated, maintain proper hygiene. Avoid strenuous activities and sweating.

A: I don’t have any of the above symptoms currently or in the past either it’s really only the burning sensation that I get when I urinate. I never used to have it though but for some reason it just came on out of nowhere the only thing that I was doing around that time when it first occurred was using one of those fleshlight sex toys which I haven’t used in over 2 years but I don’t know how that would cause anything and if it were an infection it would of showed up on the tests right? Hey doctor thanks for the follow up so I went to urinate before and felt a strong burning sensation it really felt as if the sensation is coming from the bladder or prostate it’s at the very beginning of urination it burns. Is it possible that I may have a bladder or prostate problem at my age of 22? I felt a bit of pressure as the urine came out in the bladder/prostate region along with a strong burning sensation bear in mind I did masterbate this morning and i hadn’t drunk a huge amount of water today but still felt pretty painful could it may be left over semen from the prostate that’s causing the pain in the urethra is that possible?

Thanks for the follow up i’ll see how i’ll go with it but just something on a slightly related note. About two months ago after masterbating I got a really bad red rash on the left side of the penis shaft. At first I applied an antifungal cream which made in burn, I then was prescribed a plain steroid cream which helped with the redness/itchyness but it still persisted. I then was prescribed a combo antifungal/steroid cream and it cured it completely however at the same time I then had a fungal infection on the glans of my penis which I was prescribed an antifungal tablet three weeks ago and I have been symptom free for about two weeks. However this morning I masterbated and the red rash on the left side returned I’ve just applied QV lotion for now but do you think it would be worthwhile applying the antifungal/steroid combo cream again for a couple of days? Just to add as well the fungal infection I had was tested with swabs and the swab only came up with a fungal infection on the glans not the shaft so could it just be irritation from masterbating that caused it? I don’t seem to have the same issue when I am have sex though.

How long does it generally take to see improvement with this cream and should I also be using Moisturizer while using the steroid cream? Hey doctor just a follow up so the rash on the shaft has gotten a lot better and I have just switched to using QV moisturising lotion on the shaft however the head of my penis is now red but before it was fine. Hey doctor thank you so much for the follow up I appreciate it one thing however I have noticed the skin of the scrotum has become red and irritated from the hydrocortisone cream as the where I apply it on the bottom of shaft it sits on the scrotum and I think some of it’s getting in there. Is that normal since the skin there is quite thin should I just stop using it for now?
