Hey, So I Have Chronic Migraine , And The Main

default
Posted on Sat, 13 Mar 2021
Question: Hey, so I have chronic migraine, and the main problem with it is not the pain but the pervasively disabling brain fog it causes -- right now I cannot even engage in leisurely activity, let alone work for a living and live on my own, because the brain fog is so severe. When I can finally get access to a neurosurgeon to do a neurectomy on parts of my trigeminal nerve, it will rid me of the brain fog, but at this rate this is going to take a full year. So, in the meantime, I need a way to lessen the brain fog such that I have some decree of functionality in my life until then. To me this means reducing inflammation of the trigeminal nerve, in my case particularly the middle branch of the nerve concentrated around the sinuses.

How do I reduce it? And/or how do I find reliable, informative information on how to do so? Search engine results on many different search engines have yielded no good results.

Brief Answer:
Botox injections may help.

Detailed Answer:
Hello and welcome to the “Ask a Doctor “ service.

I have read your question.

Since you suffer from chronic migraine with subsequent brain fog, a good possibility to be treated before going to surgery is injection of onabotulinumtoxin A ( Botox ).

This procedure when done right is found to cut in half people who suffer from migraine or significant lessening of the symptoms and duration.

So, in my opinion, discuss with your Neurologist for this possibility of treating chronic migraines.

It didn't even occur to me when typing my question to note that I tried Botox and it didn't do anything. What else is there?

Brief Answer:
Injections of lidocaine (nerve blocks ) may help.

Detailed Answer:
Hello again.

Okay then, if you did not have relief from Botox injections, another possibility is to have a nerve block with lidocaine or bupivacaine of the middle branch of the trigeminal nerve in order to get relief until you see the Neurosurgeon.

Discuss with your Neurologist these issues.

Hope this helps. Take care.

Okay, cool! I had heard of this solution, but I was skeptical because I was only hearing of it occasionally, which often means a solution is not well documented to work (though not necessarily). Thanks much!
