Question: Hi 3 weeks ago i discovered a smsll white worm in my faeces. I got vermox from pharmacy and took as perscribed. I took again in 10 days and in 15 days and followed strict hygeine as advised. However isince then my faeces is full of peaunut lik things. I dont eat nuts. Also there is yellow muscus. Mybdoctor said not to worry but surly this is not normal?

I've gone through your details dear.

The peanut like thing is very likely larvae or egg of any parasite/worm living inside your intestines.
As a result of deworming, they are coming out.
Your doctor should have asked you to go for a very simple test called stool routine microscopy and culture to find out exactly what these particles are.

In this, a small sample of your stool is studied under a microscope.
However, don't worry.
Did you used Albendazole only or Albendazole plus Ivermectin for deworming?
And why one tablet on day 0 then 10th and 15, why not every day?
Usually it should be taken straight for 5 days.

Did the doctor prescribe the doses?
And what were your chief complaints when you first presented to your doctor?

The only complaint i had was i saw one small white living worm on my stool. The medicine i took is called vermox. Yes the doctor prescribed this dose, it also states this on package.
But now i have itching and yellow mucus in stool and these peaunut type things. No the doctor didnt ask me to go for test

Inadequate deworming.

Detailed Answer:
Hi again my dear patient,
The standard dose schedule of Mebendazole is 100 mg tablets two times a day for 3 days.
I don't know why the doctor administered the second dose on 10 th and 15 th day.
Very likely the cause of seeing larvae and itching is incomplete deworming.
I would recommend you to take Albendazole 400 mg once daily at night time (chew not swallow) with Ivermectin 6 mg tablet for 5 days.
These are easily available on any pharmacy and complete deworming would take place.
For itch take any antihistamine tablet like cetrizine 5 mg for 2-3 days.

Mebendazole is an older one.
If the above mentioned medicines are not available then take a course of Mebendazole 100 mg twice daily for 3 days.

I dont understand because the worm i saw was small and white. This is a threadworm? And you cant see their eggs. Is there another worm that looks like a thread worm?

Without stool test how are you so sure that you don't have any pinworm?

Detailed Answer:
Hi again,
Threadworms, roundworms, tapeworms, whipworms hookworms etc are the most common worms in our intestines. Apart from that there are a number of species of worms which could infest intestines.
It's a matter of chance that tape worm appeared in your stools but that doesn't means that you have only tapeworms in your intestines.
In fact larvae eggs and many other stages of development of worms could not be seen by naked eyes and needs microscopic examination.

Secondly even if it's tape worm Mebendazole was not given as per protocol. It's always indicated as 100 mg twice daily dose for 3 days.

Thirdly tapeworm just like any other worm has many stages of development that begins from egg to larvae to complete parasite.Its not necessary that you could see every stage by your eyes.

Now the good thing is that a 5-7 days course of antihelmenthic would kill almost all worms(In any stage of development).
Therefore I have suggested you to take a revised and complete course of medication.
Mebendazole on 1 st then 10 th and then 15 th day is totally an absurd dosing and is just wastage of medicine.
The results you can see(larvae or egg) is because of the first dose(In your case every dose was first as there was a time interval of many days in between) but itching and mucous is clearly suggestive of fact that there are larvae and eggs and may be some worms still in your large intestines.
