Brief Answer:
An ultrasound KUB should be done.

Detailed Answer:
Hello and thank you for choosing "Ask a Doctor" service for your query,
Have gone through your details my dear patient,
Very likely it could be cystitis. Inflammation of bladder as a result of bacterial infection.I would suggest you to get a urine test done to rule out presence of bacteria.
An ultrasound KUB can also done to rule out cystitis.
Though it could be premenstrual pain also yet ruling out cystitis or UTI is essential.
Hope I was helpful.
Thank you.

Hello dear pain of appendix is not felt in pubic area.
Secondly appendicitis is an emergency condition so patient would run to ED due to severe pain.
Rest assured it just UTI or cystitis.

Hello again dear,
Yes you can take any over the counter analgesic like Ibuprofen 400 mg with Gaviscon tablets.
Anti spasmodic would help but they are not over the counter.

Drink plenty of water.
