Question: Hi, Asking this question to cardiologist XXXXXXX Partap.

Hello Doctor. Am a mother of one year old baby. A week back, when I was doing exercises, (moderate ex, not heavy), I felt strange pressure in the left side of my chest and it was there for sometime n later subsided with rest. Yesterday night, I was carrying my baby and walked fast for few minutes in order to make her sleep. After that, I experienced same kind of pressure. So I went to ER, my vitals were normal and doctor said that I could be discharged after taking ECG... To my shock, they said that there are some changes in ECG and they had to take Trop t test. That was the worst few mins of my life and I was panicking. Since my brother in law is a doctor, I sent him the photo of ECG taken. He said that it is lead reversal, avr positive and that the ecg is incorrect. So he said that ECG must be repeated. He spoke to my doctor via phone. Doctor accepted to retake and the second one turned out to be fine.

Please review both the ECGs. Am still unable to get out of it. Let me know if I should go for further evaluation. The ECGs are marked I and II in the images.

First of all am glad and thankful that you showed your trust on me.

Now, I have reviewed both the ecgs and yes the first ecg is of lead reversal as we don’t have inverted p wave in lead I unless there is dextrocardia and also avr is slightly positive.

Your second ecg is perfectly alright. However as you had symptoms of chest discomfort so it’s advisable to seek medical opinion by visiting a cardiologist personally and getting a stress echo done under his supervision to rule out any underlying issue.

Thank you, Doctor. I consulted a cardiologist and did ecg again n tmt as well, the results of which I shall post it tomorrow. Doctor, you said that avr is slightly positive. What is that? Is that a problem?... Doctor, last year when I did an echo as a routine check up, they said that I have MVP with trivial MR /TR. My doctor brushed off that too saying it is common among women... Please brief me about that. Will it cause any problem in future?

Yes MVP (improper closure of mitral valve leaflets) is common and most patients usually don’t require any treatment until unless there is any significant valvular regurgitation (leakage of valve). As in your case there is trivial MR so you need not worry as of now. However it’s advisable to keep getting a follow up echo every year.

Thank you, Doctor. I've attached the reports of ECG n TMT test that was taken yesterday. Kindly review the same. Sorry I didn't take the stress echo. After reviewing the report, please let me know if I should still go for it.

And Doctor, I have 2 more questions. You said that avr is positive in my first ecg. Kindly explain what is that. And what are the chances of regurgitation getting worse in future due to prolapse?...

Your TMT report is alright, don’t worry.
When there is wrong placement of leads while doing ecg the graph deflections changes, avr lead should have a downward deflection ideally, if that’s upward which is not the usual scenario it’s called upward or positive deflection.

Regarding progression of MR it’s difficult to predict the future that’s why follow up echos are important.

Thank you Doctor. Can do I exercises normally or should I avoid any specific exercises?... I live with constant anxiety that something will happen to me if I exercise too much... And do I still need a stress echo to be done?
