Question: Hi Doc,

Below are my semen analysis parameters. Can you please let me know if they are normal.
Also, should I repeat the test?

Volume —— 2.5 ml
Liquefaction time —— 65 mins
pH ——- 7.5
Fructose ——— present
Sperm count ——— 44.3 million / ml
Total motility (progressive + non - progressive ) ——— 70%
Progressive motility——- 50%
Sperm with normal morphology ——- 55%
WBC ——- 8-10 hpf
Epithelial cells —- NIL

The rest was done in General hospital and not a specific andrology centre.

Your semen report is normal except for mild infection

Detailed Answer:
Dear Vik,
Thank you for your question. All your semen parameters are normal except for delayed liquification, that indicates how much time the semen takes to form a watery form, usually it is less than 30 min, however in your case it is more than a hour. Also, there are Pus cells which is another minor issue. Bother the issues may be due to mild infection. Usually in these cases we give a course of oral antibiotics and repeat the test. Also, starting antioxidant treatment will be a good idea.

Thanks for your quick response and detailed explanation doctor.
Yes, I have been trying for pregnancy since 10 months,
I thought to get myself tested to know if there is any problem.

Since the above test was done from a general hospital, I was not sure about the results.
So I got one more test done from an andrology lab who deal specifically with these kind of stuff.

Below are the results

Volume : 1.8 ml
Liquefaction time : 20 mins
Viscosity : Normal
Appearance : Grey- Opaque
pH : alkaline

Sperm concentration : 34 mil / ml
Total count : 61.2 million
Total motility : 53%
——— Progressive: 39%
——— Non- Progressive : 14%
——— Non motile : 47%

Morphology
Normal : 2%
Abnormal : 98%
Head defects : 38%
Neck and mid-piece defects : 42%
Tail defects : 18%


Cell debris : Present
Leukocytes : Occasional
Epithelial cells : Occasional

IMPRESSION : TERATOOZOOSPERMIA

I am quite worried with the test result, Is there any problem with the above test.
Do I really need to worry about the teratoozoospermia and any treatment available doctor ?

Dear Mr Vik,
Thank you for your follow up questions. I have gone through your semen report, and unfortunately it stays the structural appearance of almost 98% sperms are abnormal. In very few infertile men we do see such reports. As you are in younger age and just started trying for pregnancy since 10 months, kindly do not lose hope. Due to advances in infertility technology many things can be tried.
Firstly, I advise you to get evaluated by an infertility specialist or an urologist to know what is your testicular volume, is there any gross abnormalities like small volume testes, or gross vericocele, and the doctor may order any further tests like ultrasound of testes or any genetic tests for you.
One it is done , we usually start such patients with a course of tablets antioxidants and multivitamins for 2 to 3 months and repeat your semen analysis once again. Then the further management depends on your test results, whether to go for IVF ICSI or Testicular sperm aspiration and then ICSI i.e where good healthy motile sperm is injected directly into your partners egg to get good results.

I will consult an urologist and get the respective scans done at the earliest.

Had last few more questions doctor, sorry to bother you.
1. Is Doppler test of penis and testes mandatory in such case? Or only ultrasound would be enough. I had read it some where.
2. How does weight play a role in the sperm morphology figures, since I am obese and currently weigh 115 kgs (having lost 10 kgs till date). Is it clinically right to say if I reduce weight and maintain a healthy lifestyle, can things turn around. Or is it more of genetics which I have to live with.
3. And these are my testosterone parameters: are they normal?
Free testosterone : 7.14 ng/dL (Ref : 5.7 - 17.8 ng/dL)
Bioavailable testosterone : 164 (Ref : 125.5 - 411.8 ng/dL)
Free Androgen index : 41% ( Ref : 35.9 - 92.6% )
4. With teratosperima, IUI would not be beneficial ?

Dear Mr Vik,
Thank you for your follow up question.
1. Ultrasound and Doppler is only applicable after getting evaluated by an Urologist. Usually they will examine your testes and decide if it is required.
2. Definitely weight is an important factor for abnormal semen parameters, obesity is usually associated with abnormal semen parameters. Lifestyle changes like loosing weight, (avoid smoking or alcohol or any recreational drugs if being taken). Take lot of vegetables especially tomatoes and stick to healthy diet, avoid stress.

3.Your testosterone levels are normal.

4. Yes, you can try IUI , but I strongly suggest you to change your lifestyle, reduce weight, start with antioxidants and repeat the semen parameters after ideally 3 months and then go for IUI. Definitely you can try IUI for few cycles as it is less invasive and inexpensive compared to IVF.
