Question: Hi Doctor -

It won’t let me write a followup.

Thank you so much for your input! I’m trying to get this out of my head but when I feel like pills and food gets stuck in my throat it puts me in a panic and convinces me I have bulbar ALS. I keep thinking that because the barium test is only liquid it wouldn’t have shown if food gets stuck. I’ve read online in ALS blogs that people presented with my same symptoms when diagnosed, this is what scares me the most. ALS doesn’t seem so rare to me, I live in NJ and read 400 cases in the last 3 years! My dr put me on sertraline, venlafaxine and xanax because of the anxiety this has caused me. He feels it’s globus sensation- wouldn’t that mean I feel like something is stuck all the time? I feel it after eating and taking my pills only. Please any other input would be much appreciated. Are you 100% confident I dont have this horrible disease?

If you had bulbar ALS, during the barium swallow, the liquids would not have been swallowed to allow for the testing without interruptions. Bulbar ALS actually causes difficulties swallowing as detailed in the response #2 on our last correspondence.

Bulbar related dysphagia is not about having a sensation in the throat. It is about being unable to initiate swallowing. This is most pronounced with liquids. This is not what you have. Do yourself justice and stop worrying over this. You are really not a candidate I will pursue further evaluation for ALS at this time.

So what is causing me to feel the pills / food being stuck?
I spoke with my GI dr and he said there was no reflux noted on the barium swallow. Why would my esophagus have dysmotility and not have synchronized contractions if not caused by reflux? What else could be the case? Can stress / anxiety influence it?

Dysphagia is a common symptom but in most cases not linked to ALS. I will like you to focus on getting the dysphagia evaluated. Uf you have a GI, he should consider you for EGD. If EGD shows all is fine, then this would be considered a globus sensation and managed as such.

Anxiety and stress are your biggest enemies at this time. I know it os hard but if you could actually avail yourself from thinking you have ALS, you would do much better and symptoms would significantly decrease.

I was started on Zoloft and Effexor 10 days ago which makes me feel like my mouth is so dry. I have mucous too. Can this contribute to my issues with pills / food getting stuck? He mentioned in the barium exam that the oropharyngeal and larynx function were normal. Swallowing was normal (they tested liquid only).
It’s very difficult to not think this is bulbar onset ALS as I have done so much research online and read about patient personal experiences and feel like my situation is so similar

Ok thanks so much Dr for your reply, I certainly feel better. It was patient blogs I was reading and this lady my age had similar symptoms and was diagnosed with Bulbar ALS- that sent me into a panic. I also want to mention I went to see an ENT who pit the camera through my nose last Tuesday and stated that he saw nothing, everything looked normal. Is it really possible that all these issues are from my anxiety/stress. This all started when I had a sore throat for two weeks in April and got scared it was COVID19 then I thought maybe its from Bulbar ALS and have been fixated on it since then.

Just now I ate an apple, right after I burped and felt pieces of the apple coming back up. This is what’s scaring me. Doesn’t this happen with bulbar ALS? It just feels stuck in my throat.
Can my anxiety be making my throat muscles tight so the food doesn’t pass?
