Hi Doctor, Accidentally I Swallowed A Few (3 To 4)
Accidentally i swallowed a few (3 to 4) drops of breast milk from a women who is having galactorrhea( which i confirmed by asking her in detail).
I dont know her HIV status so i tested a antigen antibody test at 18 th day the result was negative with a value of 0.2.
On 46 th day i again tested again the result was negative and the value was 0.226.
Now my query is why there is a difference in the value do my behaviour has a potential risk for infection.
Please relief my doubt as soon as possible
You are free from HIV 1 infection. Your HIV risk is negligible
Detailed Answer:
Hi,
Welcome to ASK A DOCTOR through HCM.
Thanks for posting your query.
First of all, there is every likelihood for that girl could be a HIV negative one. Then this question does not arise at all.
Transmission rate of HIV through breast milk is very low than other body fluids like blood, semen and vaginal fluid etc.The chances of HIV transmission through breast milk for a baby fed on breast milk for months is only around 10-15% only. In spite of continuos breast feeding for months together nearly 85-90% of the babies would escape from the infection in spite of the mother is having the infection. Sucking the breast onetime is almost having no risk for HIV transmission.
Moreover if we consider that girl is HIV positive, even then, if she is on ART drugs and her viral load is undetectable, then also she remains totally non-infectious. So there is no chance for HIV transmission.
Regarding your test:
You had antigen and antibody detection test, that is 4th generation test. It has two components. The antigen part can detect only HIV 1 and not HIV 2. This part of test can be considered conclusive by 3-4 weeks. So a nonreactive test 46 days after the encounter ruled out the possibility of HIV 1 infection.
The antibody detection part can detect both HIV 1 and 2 whereas the window period for antibody test is 4-6 weeks, rarely delayed upto 12 weeks. Only 12 weeks after the encounter, we can consider the test is conclusive one.
The value mentioned by you are less than the 'cut off' value of that particular test. The value has no clinical significance. You need not worry about the difference of test value. Whether it is less than the cut of value or not, that only is important.
