
Hi Doctor, I Am A 62 Years Old Male, Considerably

I am a 62 years old Male, considerably healthy with active life style, regular physical workouts, 68 Kg.
Gall bladder removed in 2005.
I am having irregular bowl movement, constipation, hard stool for few months.- started in XXXXXXX 19.
4th March - had a severe lower right back pain, difficulty in bending, even putting on socks or shoes.
consulted physician, blood test had some infection. was given antibiotic and flagyl for 5 days.
after 7 days pain was back (not as much as before).
went for Ultra sound - Report "Right Iliac Fossa Inflammation and hence clinically consider underlying Appendicitis. No other significant findings - other than Fatty Liver".
Was advised to have Appendics removed.
Second opinion - was advised to get CT scan.
CT Scan on 23.06.19 reports - "Essentially unremarkable CT scan study of the liver, spleen, pancreas, kidneys, adrenal glands and urinary bladder. s/p Cholecystectomy, No evident appendicitis or peri-appendiceal abscess. No ascites orlymphadenopathy".
Current situtation. not on any medications. sometime still get the discomfort in bending, specially in the morning. slight pain on right back persists. Irregular bowel movement, Hard, smelly, bloated belly.
Please advise.

I saw the Ultrasound and CT scan reports.
In Right iliac fossa,there will be Appendix, Terminal ileum,Caecum.Apart from Appendicitis, Inflammation of the Terminal ileum and Caecum can also cause pain in that area of Right iliac fossa.But,they doesn't cause pain in the back unless Appendix develops Abscess and pus is collected in the back.But,no.abscess as per CT scan.
Right sided Ureteric stone can cause pain in the back ,but it is also ruled out.
As there is visible pathology inside the Abdomen,the next possiblity is Musculoskeletal pain.
Please stop all the workouts for few days.
I prescribe Skeletal muscle relaxant tablet containing Thiocolchicoside along with Anti inflammatory tablet like Aceclofenac to my patients with similar complaints.
Response will be good with this combination.
Please take high fiber diet.Take frequent small feeds.Stop Non vegetarian food completely.Drink plenty of liquids.
Outside food should be avoided.A pre and probiotic capsule may help.
Nothing to worry.
It will come down.

For the last 3 months, i have slowed down on exercise. Completely stopped for 3 weeks, and 2 weeks to see any change. But has not helped. Though did not take pain killer as i am allergic to many of them. I can take paracetamol only. Brufen, aspirin, voltarene had reacted.
My other annoying problem is irregular bowl movement. Constipation, gas and bloating, which is very discomforting.
Could you please advise on this.

If you are allergic to Analgesics, you can take Tablet containing Thiocolchicoside only.It is a skeletal muscle relaxant.It cones in two doses of 4mg and 8mg. Depending upon the severity,dose is given.
Regarding bowel symptoms, you should take small frequent feeds.I prescribe a Pre and probiotic Capsule to my patients with similar complaints.As CT scan is normal, disease inside the large intestine is ruled out.

Just for your info I had taken the following mediciations since the problem started.
Medicines

1 Ceftriaxone IV nosocef 1gm 1x 5 days
2 Cephadex – cephalexin 500mg 1x3x7days
2 Flagyl 500mg 1x2x5 days
I had started taking aclock for the last 2 days and it has given good relief at least for gas.
I have started to follow the diet as per your suggestion.

The images I have is on CD and can only be read/run from CD.
Converted to PPTx but this site does not allow this file type.
I can convert to JPG/TIFF but there are thousands of file Total 566mb.

Flagyl contains Metronidazole which is used to treat anaerobic infections, Amoebiasis etc,It causes Gut disturbances like constipation, abdominal discomfort etc.Please stop that.
I saw the blood reports.Only once,blood is seen in the stool.It can be due to many conditions like Infection, Fissure, Haemorrhoids.
As the CT scan report is normal, there is no need to Upload the images.
If the symptoms still persist after Thiocolchicoside,then I prefer a Colonoscopy test for you.Cause of bleeding per Rectum can be known.It was seen in First sample of Occult blood test.
