Hi Doctor I Am Not Sure If You Remember Our

Question: Hi Doctor

I am not sure if you remember our conversation was about 6 weeks ago, I had an exposure with a woman involving deep kissing, i did have a cut on my mouth but no blood, I was advised by you not to worry and for a peace of mind to do another test at 12 weeks, I did a Test for all STDS (chlamydia, Gonorrhea, syphilis), they came back negative, I also did a test for the following:

Hepatitis B Antigen- 4 weeks, 6 weeks, 9.5 weeks and 12 weeks - NOT DETECTED

Hepatitis C Antibodies- 4 weeks, 6 weeks, 9.5 weeks and 12 weeks - NOT DETECTED

HIV Test - 12 weeks - NEGATIVE

I have a question, is 84 days conclusive for Hepatitis B, some sources say 6 months, , However my doctor and the CDC website stated 3 months is more than enough time for the antigen to have shown up by now

from my understanding according to wikipedia the reason why some people say 6 months is because they are referring to the period that elapses during HBsAg to HBsAb seroconversion, i.e. between the disappearance of surface antigen (HBsAg) from serum and the appearance of HBsAb (anti-HBs), if am correct?

Brief Answer:
Very likely you do not have hepatitis, a last check at 6 months is recommended

Detailed Answer:
Hello and welcome to "Ask a Doctor" service,

I carefully read your query and understand your concern.

Most of the time, hepatitis B becomes detectable within the first three months of contracting the infection. There are a few cases that it may take up to 6 months though. This means, in some cases, it may take up to 6 months for HBsAg to show up in the blood (according to the World Health Organization).

I would say that because the blood tests so far are negative very likely you do not have hepatitis B. You will need to repeat the test in 6 months from the event to make completely sure. You do not need to repeat the test in between from now to when it is 6 months. I noticed you had the tests very often recently.

On the other hand, please be reminded that even when adults get infected with hepatitis B, more than 95% fight the infection and do not become chronically infected. In your case chances are very slim you have contracted the infection and even if you got the infection it is only 1 in 20 the risk to remain infected in 6 months.

To conclude:
- very likely you are safe and do not have hepatitis B infection
- to completely rule out hepatitis B infection another test should be performed at 6 months (according to WHO)
- only 1 in 20 adults who contracted hepatitis B remain infected in 6 months, so there is a 95% chance that the infection will be naturally healed by your body even if you are infected
- a very good thing in your case is that you have awareness and are doing regular checks to make sure if you have the disease or not. I would recommend just to follow up with this last step of confirmation in 6 months (from the day of the event).
- if you get a negative result in this time you may need to have the Hep B vaccine series to gain lifetime protection from hep B infection

I hope this is helpful.
I remain at your disposal for further medical assistance.

Hi Doctor

Thanks for the reply, I had a look at the World Health Organization, it states the following

"The virus may be detected within 30 to 60 days after infection"

I do understand there are rare cases of it becoming positive, I hope its conclusive for me

I did speak to the girl that I was exposed to, she told me she doesn't have any std she had just been tested last week for Hepatitis B and C which came back Negative, yet I find it hard to trust people

The reason i am so concerned is because of joint pain in fingers, toes, knees etc

I also developed blood vessels that started appearing next to my nails, they look like spider veins red lines

I just want to know if it was an active hep B infection that was causing these symptoms would it have shown up by 12 weeks?

Brief Answer:
I do not think the Hep B infection is the cause of your joint pains

Detailed Answer:
Hello and welcome back,

I carefully read your query.
If the girl is negative for Hep B it is almost unlikely for you to get hepatitis B. Also because your blood tests for Hep B come out negative, it is unlikely the symptoms you are having are due to hepatitis B. So, I would like to reassure you that you do not need to worry about this.

To conclude:
- I do not believe the joint pains you are having are due to hepatitis B. All your blood tests do not detect HbsAg which means it is not increased in the blood and the virus can not be affecting your body. So, hepatitis B can not cause symptoms if it is not reproducing in your body.
- if the woman you had the encounter is Hep B negative this is another reassuring factor

I hope this is helpful.
