Hi Doctor: I Just Had My MRI Completed With Contrast

Question: Hi Doctor:

I just had my MRI completed with contrast and received certain findings that I am confused about. About 1 week ago I had a CT scan with contrast and nothing showed up as concerning. Both were done to see why I have microscopic blood in my urine.
In the findings area:
Liver/Biliary: There is a patchy drop out of signal within the liver on out of phase imaging. No bile duct dilatation or heptic mass. Gallbladder appears normal. Can you please tell me what that means? On my CT scan everything showed as normal in that area but I am not certain if that is worrisome or what is going on with that.
Kidneys/Ureters: There are bilateral symmetric nephrograms. A simply cyst within the midpole of the left kidney is noted measuring 4 mm. There are no suspicious renal mass is identified. The ureters are normal in caliber and demonstrate no filing defect. Can you please explain what all of that means and if anything is worrisome. On the CT scan everything came back as normal. I have no idea what bilateral symmetric nephrograms is about and it is troubling (to me) that they found a cyst.
Impression: No specific MR findings are identified to account for the patient's hematuria. No mass or filing defect identified within the ureters or bladder.
Can you please let me know your thoughts of the radiologists remarks about the findings identified above? My Doctor has been busy with day to day and never returned my (rather frantic) phone calls and emails.

Thanks, XXXXXXX

Indication: Asymptomatic microscopic hematuria

Comparison: CT abdomen and pelvis dated 1/29/2019.

Technique: Pre and postcontrast enhanced MR urogram of the abdomen and pelvis. 20 mL of Dotarem was administered intravenously.
3D reconstruction was performed on a separate workstation and images were sent to PACS.

FINDINGS:
LUNG BASES:
No significant abnormality.

Abdomen/Pelvis:
Liver/Biliary: There is patchy dropout of signal within the liver on out of phase imaging. No bile duct dilatation or hepatic mass. Gallbladder appears normal.

Spleen: The spleen shows no significant abnormality.

Pancreas: No significant abnormality.

Adrenal Glands: Adrenal glands appear normal.

Kidneys/ureters: There are bilateral symmetric nephrograms. A simple cyst within the midpole of the left kidney is noted measuring 4 mm. There are no suspicious renal mass is identified. The ureters are normal in caliber and demonstrate no filling defect.

Lymph nodes: No lymphadenopathy identified.

Vascular: No significant abnormality noted.

Other: No ascites is seen.

Bowel: There is no evidence of intestinal obstruction or adjacent inflammatory changes.

Bladder: The bladder is distended and demonstrates no abnormal enhancement or filling defect.

Prostate gland: No significant abnormality noted.

Bones/Soft tissues: The visualized osseous structures show no significant abnormality.

IMPRESSION:
No specific MR findings are identified to account for the patient's hematuria. No mass or filling defect identified within the ureters or bladder.

A week ago or so they tested my blood:

AST 21 unit/L
ALT 33 unit/L
Albumin level 5.3 g/dl
UA Blood shows trace
UA WBC 1/HPF
UA RBC 2/HPF
No Bacteria

Sorry a lot of info and a lot of questions. Is that showing only 2 RBC's in my blood. Is that not considered normal to have 0-3?

Brief Answer:
Everyhting seems fine

Detailed Answer:
Hi again Crosen22000,

Thank you for the report. Everything is normal, the symmetrical nehprograms simply imply that the kidneys function symmetrically. The cyst is 100% benign and is of no significance.

Monosymptomatic, microscopic hematuria is in many countries no indication for further diagnostics, as it rarely is associated with disease - just as the case with you!

Hope you find this helpful. Please let me know if you have further questions.
