
Hi Doctor, My Daughter Is 22 Months Old And Her

My daughter is 22 months old and her weight is approx. 12 Kg.
She is having following condition:
1. Fever since 20th October.
2. Cold and runny nose since 20th October
3. Cough started on 21st October and now has become bad and worsens when she sleeps. During the day it is kind of Ok but, during night, she coughs a lot.
We have given her following medications since 20th Oct:
1. Montair LC kid - 5 ml once a day which we are still continuing
2. Calpol - 1.8 ml SOS or 6 hourly
3. Maxtra - 2.5 ml 3 times a day
4. Ventorlin - 3.5 ml 3 times a day for past 2 days (since 23rd October) as she was having whooping sound in cough and prescribed this medicine earlier in same situation
Now, fever seems to be decreasing and we don't have to give calpol that frequently but, cough is bad and worse at night. Also, apart from above medicines we have Ambrolite - S available at home in case we need to give that. Please suggest the cause and remedy. Thanks in advance.

I have gone through the query and do understand your concern.
Most common cause of cough and cold with fever in this age is some viral infection.
Fever remains of high grade during initial days and then it starts declining but cough may persist longer.
The cause of more cough during nigh time is comparitively more narrowing of airways.
Ventrolin contains bronchodilator and helps in relieving airway spasm. Maxtra contains decongestant and antiallergic and gives symptomatic relief.
Montair LC also contains antiallergic montelukast which is longer acting and levocetrizine which is short acting. I don't think any advantage of giving montair LC with maxtra. Usually montair LC is given for prolonged condition or for maintenance once acute condition subsides.
With the same treatment I hope the symptoms will be better within next 3-4 days. You may give the evening dose of Ventrolin little later for its better effect during night time.
Ambrolite s also is a bronchodilator and it is not needed if we are already giving Ventrolin.
If you notice any fast breathing, vomiting following cough or any significant problem please contact your pediatrician immediately.

1. As you said if you notice fast breathing following cough then contact pedictrician......after cough she generally breathes a little heavily which is audible but, there is no vomiting associated. Is this normal or do we need to see a doctor physically?
2. Do you think nebulizing her will help in this situation? She was suggested nebulizing in an earlier almost similar situation with Budecort rapsules (2ml).

Steroid nebulisation is not suggested in viral respiratory infection

Based on history to me it seems to be viral respiratory infection as it is associated with fever.
Budecort is a steroid and although many physicians prescribe steroid nebulisation(as it improves symptoms) , it is not the recommended treatment.
Personally I would like to observe with the same treatment. If fever is completely gone and there is no improvement or worsening of condition steroid nebulisation can be given after consulting your pediatrician.
Mild breathing difficulty if common after heavy cough but if there is baseline increase in respiratory rate or chest retraction is there (best assessed while sleeping) please contact your pediatrician.
