Question: Hi Doctor,

This is XXXXXXX I'm pregnant (7Weeks, 2 Days). Here's the report copy of all the blood tests for your review.

Age : 27
Pregnant : 7Weeks, 2 Days
Thyroid : Hypothyroidism
Current Medication : Thyronorm 112.5mg
Started mcbm69 : 5th Nov 2018

My Doctor (OBGyn) advised to continue the same dose of Thyronorm after seeing the attached report. However; I'd like to be doubly sure and hence, like to seek your advise if the same has to be continued or should there be change in the medication?

I have taken 2 shots of hepatitis B (government campaign recommended 3 doses ) and 3rd is pending to be taken on 18th Nov.

1st Shot : 9th Sep
2nd Shot : 14th Oct
3rd Shot (Pending) : 18th Nov

Should I take that or skip the 3rd shot?

Thanks in advance.

Brief Answer:
need to increase the dose.

Detailed Answer:
dear madam, hi! I must congratulate you for the very wise decision of consulting a thyroid specialist for thyroid related problem. Again you are right in thinking the dose needs a change in view of pregnancy. Pregnancy targets are bit tighter than non pregnancy state. so for the duration of pregnancy we try keeping TSH around 1 and FT4 around 1.5. So in keeping with the targets, we need to increase your thyroxine dose from 112.5 to 150 mcg . also during pregnancy we need a frequent monitoring and frequent dose adjustments. so we would suggest a repeat check up of TSH and FT4 after 4 weeks of increasing the dose i.e. tomorrow.
Another thing i would like to mention is that your fasting blood glucose is 93 mg/dl. this is also a bit high for pregnancy. kindly get a GTT and Hba1c done and let me know.
regards!
