Hi, Dr. Chail: I Was Unable To Delete The Previous
I was unable to delete the previous reports in order to make room for more reports - I wrote to you asking about the cervical mri for neck pain, throat, and vertigo, and with the torso mri. Yes, the back problems have caused severe life function limitations and problems; limited range of motion, unable to sit in most chairs without pain and subsequent falling to the ground, or unable to stand up straight after sitting, unable to stand in place other than for brief periods of time, unable to run, exercise (other than walk and slow swim), unable to lift, twist, turn without extreme care, unable to lift a gallon of water or milk without extreme care and using the legs to help carry it, unable to carry groceries unless the bags are just a couple of pounds. Symptoms include falling to the ground (the back just gives out), chronic pain (made worse with activity), severe aching (made worse with activity), some burning sensations sometimes, some tingling and numbness sometimes, constant exhaustion, small time frame of stamina (2-3 hours of functioning) then having to lay down with knees fully bent to regroup, and be able to be active another few hours. Things that relieve the pain and limited mobility include: bending forward, or stretching forward, laying down with knees folded (laying flat is not possible without severe pain and inability to stand afterward and laying with knees elevated does not help - the ankles must be elevated as well or the knees folded as close tot he body as possible), walking and slow swimming helps but the swimming cannot include the up/down leg strokes, the slow frog stroke or yoga/tai chi like movements in the water help, steam/sauna/hot water jacuzzi help, but now with Covid all of these facilities are closed so the back symptoms and pain and corresponding disabilities are increasing quite quickly. Can you please give me your opinion on the bone, disks, arthritis, and facet joints? Or perhaps is there something else? Thank you for your help.

There are mild degenerative changes in the spine
Detailed Answer:
Hi,
Thanks for writing in to us.
I have reviewed the given images.
In the cervical spine, there is suspicious anterior subluxation of C3 over C4 vertebra. There are mild osteophytes in C4, C5 and C6 vertebrae. There are mild facet joint degenerative changes.
There is disc desiccation in the cervical disc levels. There is minimal posterior thecal indentation in C4-C5, C5-C6 and C6-C7 cervical disc levels. There is no spinal canal stenosis.
In the thoracic spine, there is a small area of focal bone edema anterior inferiorly in a mid thoracic vertebra and might be the cause of pain. In a level below that we find a mild posterior disc bulge. There is no spinal canal stenosis.
In lumbar vertebrae levels there are mild osteophytes. There is mild posterior listhesis of L3 over L4 vertebrae. There is disc desiccation in L3-L4, L4-L5 and L5-S1 disc levels.
There is minimal posterior disc bulge in L1-L2 and L4-L5 disc levels. There is mild posterior disc bulge in L2-L3 and L3-L4 disc levels and suspicious spinal canal stenosis. There is possible bilateral neural foramina compromise.
You are having features of mild degenerative changes in the spine and this is a difficult time to continue with out door activities due to pandemic. Please discuss with your doctors if you can be provided with physical therapy at home.
Regards,

Thank you very much for your helpful information. Just another question, in the axial images "1 Thoracic and 17 Thoracic" the disk appears to have a bubble or little lump protruding off of it on the bottom left side, is this anything?

It is a mild disc bulge in the thoracic disc level
Detailed Answer:
Hi,
Thanks for writing in to us.
The small area is a mild disc bulge in the thoracic disc level. It is not causing any serious problems. It cannot be cured completely. Physical therapy and avoiding heavy activities or stress on the spine can prevent further worsening.
