Question: Hi Dr
I’m experiencing extreme burning pain down inside my esophagus in my chest. I was told by a Dr 3 1/2 years ago my esophagus was damaged by too much drinking. At the time I was getting Therepy for rape and abuse so they put me on Gabapentin to help with the anxiety and help to not drink. I haven’t drank but my symptoms in my esophagus are worsening lately. (Especially when certain foods are eaten ). But it’s not just regular heartburn, it’s more severe and further down to the bottom of the esophagus

Brief Answer:
An upper endoscopic examination might be needed- some advice on handling symtoms

Detailed Answer:
Hello and welcome to "Ask a Doctor" service.

I carefully went through your query. I am so sorry for your past and what has happened to you.

As for the burning at the lower level of the esophagus it can be due to reflux (GERD) but because of the history of alcohol drinking and previous damages it is best to have an upper endoscopic examination with a camera and a tube that go through the mouth, esophagus, stomach and first part of the small bowel.

This will identify if there is redness present there and or other lesions like ulcer or mass.

At the moment you can take Pantoprazole 40 mg, one pill 30 minutes before breakfast and try to avoid those food that aggravate you.

Smoking (even passive smoking), coffee, fried food, tomatoes, acid food and spicy ones are the ones that make most of the patients worse.

You can have fruit and vegetables (the ones you feel ok with) in frequent small amounts and other food that you tolerate well (in frequent small amounts) and water (avoid sodas, energy drinks, coca, etc) in frequent small amount to reduce reflux from the stomach to the esophagus.

You can raise the bed head with bricks to not be flat when you sleep (do not increase the number of pillow as this may hurt your neck)

I hope this answers your query. Let me know if I can assist you further.
