Question: Hi, I am 22 years old and something wrong is going on with me and I don’t know what. I have routine tiny pressure release sensations that feel like tiny blood vessels giving out in my ankles, legs, and torso. I do not believe they are muscle twitches as I cannot see any movement from looking at my skin and they are very localized. Could this be a nerve issue? How would I know if it is a vascular problem? It has been going on for a few years now and sometimes my legs feel sluggish and weak in the mornings. I have not told my PCP about this yet. As well, a few years ago, one night, I laid down to go to bed and something that felt like a balloon burst on the front left side of my chest under my lower side ribcage, under my breast. Doctors found nothing on X-ray. I also had a few night sweats in the past. Could it have been a cyst that burst? I ask because I get swollen lymph nodes sometimes behind my ears, under my arm, and groin. I also sometimes continue to feel a suctioning sensation in the same area where it burst. Along with that, a few days after that, I felt fluid in my chest that rolled down my torso as I sat up in bed one night and I started wheezing. I wish I would’ve gone to the ER, that night but I didn’t, and my breathing changed forever after that event. I was 17 then. Since then, I cannot breath in steam, as when I am in the shower or from a humidifier, without feeling like I am suffocating and sometimes it is harder to breath when I lay flat. That night, I laid wheezing with fluid in my chest the whole night and the next day, while walking I suddenly had some sort of spasm in my chest in the center. It felt like something narrowed or tightened near my heart and I could feel my heart beating really fast as it happened. I almost fell forward, but the spasm stopped. It lasted about a few seconds and was very frightening. Afterward, I had a brief ache under my left arm. My bloodwork, EKGs, and X-rays all come back normal. I also had a normal CT scan and echocardiogram over 3-4 years ago. At that time, I was diagnosed with mitral valve prolapse and regurgitation but a mild case. At times and over the 4-5 years since then, I have been told I am anemic and with low blood pressure during routine doctor’s visits, however during my recent visit a few weeks ago, neither of those conditions were present in my bloodwork. I also belch a lot more and my chest/ upper abdomen feels discomfort. The chest and abdomen discomfort ranges from a soggy feeling, to wetness, to a ballooning or gas moving around feeling and sometimes it feels like there’s a knot in my upper abdomen and at other times like something is being pulled or stretched or a muscle quivering. Is it the muscles my chest wall maybe? Lastly, I almost fainted last month while in public. Please give some advice. I have seen a pulmonologist and cardiologists. Should I see an oncologist next? I do urinate every time I drink something. Could I have cancer and not know it? Should I get an mri of my chest and upper abdomen maybe? I don’t know what to do anymore.

Has your thyroid levels been checked in the recent past?
Did your clinician go for the work up of your anemia including Peripheral blood film,serum folate and vitamin B12 levels.

Kindly share your Blood counts,blood sugar,Vitamin D,serum calcium and serum electrolytes levels (if at all,they were procured)

As your CT chest is normal,there is no need for MRI Chest because a CT is more informative then MRI as far as chest study is concerned.

You can go for a PET Scan of the body to detect how your body tissues and organs are functioning.

Take soft easily digestible diet including plentiful vegetables and fruits,have plenty of oral fluids and water,avoid caffeine and alcohol,de stress yourself,get sun exposure,be physically active and avoid sleep deprivation.

I have uploaded all I have. The bloodwork is my most recent from 2-3 weeks ago. The skin pictures are marks I keep getting on my center chest and side of neck. I am not sure what they are. I honestly think I have endocarditis. I say this because I used to have night sweats as well and my chest discomfort has never fully gone away, neither has my breathing gone back to normal. I also had a root canal done in the past and never finished taking my course of antibiotics after the procedure. I had a dentist appointment last year with a new dentist, and he didn't see anything except a few cavities in my front teeth, so I'm assuming he would have informed me if there was an infection in my back root canal tooth from the dental Xrays, correct?

I also attached the ultrasound of my right heel as that is the heel I stepped on something of unknown material that was tiny in 2014 and I couldn't see it to remove it, yet could feel the pain every time I stepped down. Shortly after that incident, and after the foot started sweating and throbbing, is when I started having the vascular pressure release sensations in my legs and then eventually my torso. I have had several Xrays on the foot and an MRI in 2016 and nothing showed. The only test that showed anything was the ultrasound in 2016. I also hate that I stepped on something that was in my carpet, as I have always had bad allergic skin reactions to the carpet in my house and was diagnosed with dermatographia at a younger age.

I also used to have shoulder muscle problems and went to an orthopedic once in 2014 and once again in 2015 who took X-rays of my right shoulder that was bothering me. I never told anyone, and I may be mistaken, but my heart looked slightly larger to me in the 2015 x-ray than the 2014 X-ray, around when this all first started.
Furthermore, I am not sure where to go to get a PET scan or how to ask a doctor for one. I would like to get the following bloodwork done:
- blood cultures for bacteria and fungi
- lacate
- endotoxin
- aerobic and anaerobic cloud cultures
- C- reactive protein (CRP)
- PT and PTT
- PCT
- CPK
- BNP
Can you tell me what you think? Would this bloodwork be sufficient if it were endocarditis? If not, how would a doctor know if I had it or any infection/ disease of the heart valves? What diagnostic tests would identify the condition? Would a standard transthoracic echo show it, because that is what I had in 2016, or would I need a transesophageal echo or angiography? I am scheduled to get another transthoracic echo soon and my first stress test. Should I see a neurologist?

Lastly, I want an MRI of my chest and upper abdomen because I was informed MRIs are better for looking at soft tissues and that CT scans are better for looking at bones, yet CT scans can miss smaller details. Is that true?

In closing, my main concerns are my breathing, my chest discomfort, and the vascular sensations I get in my legs and torso.

Which one is the preferred investigation,CT or MRI,depends on the organs,their structure and their density.
