I have chcekd your all given history and answers.
You are having anxiety induced headache.
You are having headache due to constant anxiety since three to four months.
The constant work on computer also is the precipitating factor for headache.
It's advisable to take rest for 10-15 minute after work on computer for 1 hour.
You should try yoga, meditation and deep breathing exercises.
Take 8 hour regular sleep and avoid late night work if applicable.
Meanwhile you should get rid from anxiety to help in your headache problem.
For that along with prescribed medication you need behavioural counselling from psychiatrist.
