Question: Hi, I am (38yrs) facing heavy menstrual cycles for many years now. My Hb goes down to 6g/dL. In my Peripheral blood smear test, it shows Dimorphic anemia. I for iron induced last 1000mg dose, after which my Hb increased to 13. But after 8-9 months it has again come down to 8. I have a small uterine fibroid. Kindly advice what investigations further required and can there be a permanent fix for my problem? Thanks.

Hi,

I understand your concerns.

Following is my reply:

1) Heavy menstrual bleeding is the reason why you have repeated drop in your hemoglobin level.

2) Problem of heavy menstrual bleeding needs to be addressed.

3) Please check with your gynecologist for possible reason and solution for this problem.

4) I suggest you get progesterone releasing intrauterine device like Mirena inserted which is good in controlling such bleeding episodes.

5) If fibroid is reason for bleeding, better to get fibroid removed ( myomectomy) by surgery.

6) These permanent remedies are better than temporary solutions of increasing hemoglobin with medicines.
