Question: Brief Answer:
Zolfresh not be taken regularly

Detailed Answer:
Hello and welcome to Ask A Doctor service.

I have reviewed your query and here is my advice.

1. is it safe to take Zolfresh for longer time?
No it shouldnot be taken for prolonged periods. It is just meant to get to normal sleep pattern.

2. is there any better and safer solution available?
Melatonin supplement is safer.

3. what could be the root cause?
The cause could be low blood sugar at night or stress/depression. Can take a hearty dinner. If stressed, then do some relaxation, yoga, breathing etc.

4. will any diagnostic test help to find out the root cause?
I dont think anything will show up on investigations. Though a CT scan of head and neck maybe done.

Hope this helps. Let me know if I can assist you further.

Hi, I am 55 year old male employed in private sector. My weight is 66kg. My diet is mixed. I do regular exercise (walking, since last 20years). I undergo annual health checkup since last 15years and all my reports are normal. (Lipid, BP, Blood sugar are ok).

Last one and half years, my sleep pattern has changed. I got up in the middle of night 2 to 3 times and find it difficult to get back to the sleep. I had consulted a General Physician about 2 months back. He had prescribed Tab Zolfresh 5mg for 10 days which worked well.

After 10 days, same problem recurred. So after waiting for another sleepless 10 days, I restarted taking Zolfresh and got my sleep back.

My questions are as follows,

1. is it safe to take Zolfresh for longer time?
2. is there any better and safer solution available?
3. what could be the root cause?
4. will any diagnostic test help to find out the root cause?

Thanks for prompt reply Dr Vaishalee.

Have following questions,

1. How could I ascertain that it is related to low blood sugar at night?

2. Could it be related to age? Normally, after what age sleep reduces and to what extent?

3. I have observed that even half of Zolfresh (2.5mg) works fine for me. Even that is not recommended for prolonged use?

4. How much do you mean by prolonged use?

5. Do you recommend any specific brand of Melatonin suppliment?

Thanks in anticipation.

Brief Answer:
Zolpidem is safe

Detailed Answer:
Hi again

1. How could I ascertain that it is related to low blood sugar at night?
With a glucometer at home

2. Could it be related to age? Normally, after what age sleep reduces and to what extent?
Yes it could be related to age especially if there are changes in brain. Since you seem to be functional and active, with normal reports and no significant medical history, this seems more likely to be stress.
Normally sleep starts decreasing after age of 50 yrs. Elderly people wake up easily on slight noise and their sleep duration is also decreased. So it becomes important for them to take a short nap during day. Sleep hygiene is important like keeping the room dark, noise-free.

3. I have observed that even half of Zolfresh (2.5mg) works fine for me. Even that is not recommended for prolonged use?
No even small doses are not recommended for this purpose. I will rather suggest that you do some yoga and breathing techniques to relax and sleep properly.

4. How much do you mean by prolonged use?
By prolonged use I mean more than 10 days.

5. Do you recommend any specific brand of Melatonin suppliment?
Brands like Meloset, Altris, Zytonin, Lifotonin, Nocturia, 3 mg or less. Keep its dose as low as possible. Though it is more effective in inducing sleep. It may have some effect in maintaining sleep.

Hello Dr Vaishalee, thanks a lot for your valuable advice.

I know I am troubling you once again, but this is my last conversation on this topic.

Yesterday, once again, I got woken up at 1.15AM. As suggested by you, I checked my blood sugar level with Glucometer, and it was 130.

As mentioned in your reply, this could be associated with stress, which I was not able to relate with my sleeplessness.

Since @ last 6 months, there has been increase in my official work load and responsibilities. I was not correlating this with my sleeplessness as this has happened many a times in past.

To conclude,

1. as suggested by you to practice yoga, breathing, can you please elaborate a little on that?

2. is there any medication and dietary suppliments for this?

Thanks in anticipation as I am closing this topic.

Brief Answer:
So its not blood sugar

Detailed Answer:
Hi again

1. as suggested by you to practice yoga, breathing, can you please elaborate a little on that?
These include doing pranayam, XXXXXXX XXXXXXX yog nidra. It increases depth in breathing. This also reminds me of decreased lung capacity with age. You can also do a chest X ray PA view and a pulmonary function test to rule out any breathing issues like sleep apnoea.

2. is there any medication and dietary suppliments for this?

Melatonin is a supplement only.

There are three medications that can be used to sleep including anti-allergics like avil, benzodiazepines like lorazepam and Z-drugs like zolpidem. Among these benzodiazepines are addicting. But people get addicted to the other two drugs also because of prolonged sleeplessness. If really needed you may take anti-allergic medicine once in a while. Though they make you sleep for longer hours but quality of sleep is not satisfying.

Good luck with your health.
