Question: Hi I am a male 29 years old.

My history
UTI infection for a number of years had pain in the anus.
I was 14 went to the doctor but i would say this pain lasted for a good 6 to 7 years.

im scared incase my prostate or SV has taken some damage:

My ESR is only 2 and my CRP has reduced to 0.9 this was 20 + when i had pain when i was ejaculating.

in 2016 i seen a lump on my testes which was actually a spermatocele which a ultrasound confirmed. The ultrasound also confirmed a mild hydro.

Upon Inspection my GP at the time ruled out the Hydro had disappeared as well as the Orchitis - which i brought upon my self as i kept reexamining. The pain has since gone.

What my questions are:

Does Sperm cele or Hydrocele effect fertility and its sperm count?
are the normal parameters of the CRP and the ESR indicate that the repro organs are back to normal??

I am due to get married next year and my questions are: Does Hydro or Spermatocele reduce fertility at all? am i normal compared to someone who doesnt have this?

The symptoms i had was a little pain in my anus, and when this elongated it was a painful ejaculation which Ciprofloxacin improved i must have had this slight pain for 7 to 8 years.

Can you please give me a detailed response please - I am worried, also as a urologist have you examined sperm tests with people with a hydro and a spermatocele together?

There is no reason to suspect that your prior issues have affected your fertility in the long run. It is true, that the longer you abstain from ejaculating, the more semen will be "ready" for the next ejaculation, but 3 months is a bit excessive, I believe a week or so would be enough.

I have seen some patients with hydrocele and spermatocele, but I have not examined sperm counts, as this is primarily done in very larges celes and first if the patient suspects (from trial and error) that he cannot impregnate a woman.

The spermatoceles can lead to a mechanical obstruction in the spermatic ducts, but this requires a very large spermatocele to COMPLETELY obstruct the duct, and this is rare. Hydroceles are often idiopathic, i.e. formed without any recognizable cause. In some cases they form as a reaction to epididymitis or orchitis, or, as in most cases, are simply a congenital malformation. They are not caused by self checking, but the absence of pain certainly suggests that the hydrocele is in regression.

The most important tip to improve fertility is to stay away from tobacco! Eat healthy and drink only moderately.
Hydroceles often disappear after some time, and it can be determined from a simple physical examination by your doctor. You can also light a small flashlight behind the testicle; a hydrocele is translucent and enables a semi-objective measure of the size of it, which can be used as a reference for future measurements.

An ultrasound can assess viability of the testes and other reproductive tissues to some extent, but my best advice is a trial and error approach; there is nothing that suggests that your fertility should be reduced in any way.

What kind of home tests do you have in mind? Ultrasound, urinalysis, blood tests, etc.?

Thank you for your advice.

My final question which i have paid the additional fee for.
I did a Spermcheck Fertility test now 3 times.

The first time i did it i didnt follow the intructions properly.
Instead of 6 drops in the sample well i did around 8 or 9 and the test was positive- which showed the sperm was present.

another test i performed i could see a grey line in the C and T line which was very faint - this had turned red slowly. But this showed a pos result. I did spill some of the reagent though by accident - would this effect it? as long as some reagent was there shouldnt really make a difference?

3rd time i did it - there was no Line but this was after 2 to 3 days and the day before i had consumed a large amount of alcohol.
the tests have shown that sperm is present when i ejaculate

it says that levels take 2-3 days to get back to normal how true is this? what are the realibilities of such tests as reviews show that sometimes they are negative and when you go to a lab they are positive.

Can you kindly give me your thoughts on this. Also if i was to take a 3 month detox and absitence and if i ejaculated would all sperms come out or will some still be stored.

I take all your advice on board and just wanted to share my home testing experience.

also all the stress im spending on worrying about this? does this effect my fertility?

i am concentrating on my diet now and getting fruits in my diet. I use to not drink much water but changing this habit as well.

This really underscores the problem with most home tests

The tests you tried probably have low predictive value, as indicated by the varying results. The best way to test it is really by going through your own doctor for a test that is send to an authorized lab. Abstinence for three months will not hurt, but it seems excessive to me. The number of sperm cells in an ejaculate is probably fairly constant, and being abstinent for three months probably won't increase the number greatly. Stress might affect your ability to get an erection, but it will not affect sperm counts. It is always a good idea to eat healthy and drink plenty of water, so keep that up!

In my opinion there is no need for prostate tests or hormone levels, as these are only surrogate measures of fertility. I hope that I can bring you some peace of mind by saying that your fertility almost certainly is comparable to that of other men your age. Only if you do not succeed in impregnating a woman by vigorous attempts over 12 months time can you consider further diagnostics. All else will bring unnecessary concerns from tests with results that are probably insignificant.

I would definitely say that you have nothing to worry about. Start on the very reasonable life style changes you mentioned, and that is all you can do.

A low predictive value means that there is a high risk of the test being inaccurate, i.e. you may have a low sperm count in reality while the test shows your sperm count is high, and vice versa. This is in line with your own experience and the reviews on the website you sent a link to.

Hi I am a male 29 years old. My history UTI infection for a number of years had pain in the anus. BIOCHEMISTRY C Reactive protein 0.9 mg/L <5.0 in 2016 i seen a lump on my testes which was actually a spermatocele which a ultrasound confirmed. The ultrasound also confirmed a mild hydro. Upon Inspection my GP at the time ruled out the Hydro had disappeared as well as the Orchitis - which i brought upon my self as i kept reexamining. The pain has since gone. What my questions are: Does Sperm cele or Hydrocele effect fertility and its sperm count? are the normal parameters of the CRP and the ESR indicate that the repro organs are back to normal?? I am due to get married next year and my questions are: Does Hydro or Spermatocele reduce fertility at all? am i normal compared to someone who doesnt have this? The symptoms i had was a little pain in my anus, and when this elongated it was a painful ejaculation which Ciprofloxacin improved i must have had this slight pain for 7 to 8 years. Can you please give me a detailed response please - I am worried, also as a urologist have you examined sperm tests with people with a hydro and a spermatocele together? I await your reply.
