Question: Hi!

I am an educated 30 year old heterosexual male in good health with no diagnosed mental or sexual problems. I have only had one sexual partner to whom I have been married eight years.

I'm sure the following will now lead this to be an interesting consult--I believe I have possibly damaged my urethra. As a bit of backstory, I began urethral sounding at a very young age; around 4 or 5 years of age.

Over the years, I practiced sounding intermittently, sometimes with years between occurrences, as it has at various times presented itself as pleasurable (sort of like the feeling of urinating after holding it awhile, only more intense). More recently, I developed an interest in stretching the smaller part of the urethra that is just past the opening (going towards to base, perhaps the navicular urethra?).

I attempted to do this with a hegar sound, but had two problems—1) I could not keep the sound safely inserted when going about my day due to the length of the sound, and 2) I was in between sizes so that it seemed neither the smaller or larger sound would actually allow me to stretch.



To solve this “issue”, I created a way to stretch using a sterile balloon. It seemed to work, though I noticed a couple things: 1) after removing the device, I experienced urine leaking for about three to four days and 2) now about four to five days post that attempt, I still have a stinging sensation about one to one-and-a-half inches from the urethral opening when urinating and also when I achieve an erection, the most concerning of which to me personally is the latter.

Also worth mentioning, I used a nasal speculum to attempt to inspect myself, and due to the slightly sharp edges, believe that I may have caused a small cut inside my urethra as well, as I felt something and noticed a very small amount of blood upon the use.

There does seem to be some internal swelling that I can feel by applying pressure externally, which has been subsiding gradually. I see no other potential signs of infection such as discharge.

I do not appear to have any sort of blockage of urine and, besides the stinging, can urinate as normal.

I abhor the idea of going to see a urologist in person about this as I do feel quite stupid (live and learn) and am very modest in daily life. But, I also would like to ensure that I don’t experience pain whenever I have an erection into the foreseeable future due to something that might have been fixable if responded to quickly.

Based on the facts that you have posted stinging sensation in penis that you have is due to inflammation of urethra (urethritis) resulting from trivial injury due to repeated sounding and other objects being placed in urethra.

Consult qualified Urologist for clinical examination in person and get following basic tests done to establish diagnosis .
1)Retrograde Cysto Urethrogram (RGU)
2) Endoscopic Examination of urethral passage and bladder (Cystoscopy)

Further treatment will depend upon result of these tests and final diagnosis.

Hello,

Thank you for your reply.

The pain went away both during urination and erection.

However, now one week later from my original message to you, I noticed a white, somewhat thick liquid that was produced. The fluid did not have a smell that I could smell. I do not believe that it could have been semen because it has been several days since my most recent ejaculation.

I noticed this fluid after I slightly massaged my penis to see if I felt any residual pain. There was a small amount of discomfort and then I noticed the white fluid.

Please advise. Thank you!

I am not too concerned about it being cleared up quickly unless it is dangerous otherwise. I am very slow to take antibiotics unless necessary.

Is it very dire that I take an antibiotic? Or will it likely subside in time with little risk?
