Hi, I Am In Sertraline 50 Mg/day And 1 Mg/day
Question: Hi,
I am in Sertraline 50 mg/day and 1 mg/day of Clonotril for OCD. While it helps in reducing OCD, following are side effects:
1. Sleep disorder: Generally don't get sleep more than 5 hours. Due to this OCD worsen as I don't tired due to lack of sleep.
2. Muscles pain: I feel very tired/muscles pain mainly in legs. Due to this, it is difficult to do exercise.
3. Recently, I have done D3 test which has come low (< 15). Physician has prescribed tablet 60000 UI/week. Is there any other way to improve D3 level quickly?
Regards,
XXXXXXX

Dear XXXXXXX
Thanks for using Ask A Doctor.
I read your query and understand your concerns.
First of all I feel taking vitamin D through injection ( Inj Arachitol) should be better at current level as it will take time through the current intervention.
In regard to OCD and best suited medication while considering sleep problem, I feel paroxetine can be used instead of sertraline. Paroxetine does not interfere with sleep and causes less muscle pain. Since this is prescription-only medication you are requested to discuss same with treating doctor.
I hope this helps you.
Feel free to write back to me if you have more questions.
Thanks and regards

Thanks.
I tried all the molecules before, but only Setraline is effective. So, I am not sure whether we shoud try changing molecule.
Do you suggest any medicines for sleep disoeder and muscels pain?

Follow up
Detailed Answer:
Dear XXXXXXX
Thanks for follow up.
I am sorry to know that there was no response with other molecules recommended in the treatment of OCD. Considering the past history of no response I feel we need to look for managing side effects rather than changing medications.
For the sleep disorder the use of Zol[pidem or any other Z drugs is first option followed by use of benzodiazepines such as clonazepam or lorazepam.
In regard to muscle pain I feel we need to look for serum electrolytes as hyponatremia can be the cause for muscle pain in your case (side effect of sertraline). If that is the cause corrective measures in form of high salt diet or use of tolvapan can be recommended.
I hope this helps you further.
Thanks and regards.
