Question: Hi, I dont know why but ever since during teens I have mild to moderate migraines before (always), during (rarely), and after (rarely) my period. When i found out i was pregnant, I had an intense migraine to the point of I couldnt see AT ALL. I lost almost half of my vision before and during the actual migraine. After that, I never had migraines during pregnancy. However, after a few mos of pregnancy until now, I have had intense migraines every couple of mos before my period. to the point of losing my vision again. then, It would come back when my migraine is gone. Right now, I havent had a period for almost three mos, (Im not sexually active after pregnancy - single mom so pregnancy is not the cause). Even though I dont have a period for the last three months - March, April and May. I had an intense migraine during March before my supposed scheduled period. I lost all vision and I couldnt see any light at all. then I vomited (always during migraine) then afterwards I didnt have any period anymore and I didnt have any migraine as of now. What could be the cost of this?

Brief Answer:
The most likely cause could be migraine with visual aura.

Detailed Answer:
Thank you for posting your query.

I am Dr Sudhir Kumar, Neurologist, and I would try my best to help you.

I have noted your clinical details. Based on this, the most likely diagnosis is migraine with visual aura. For severe attacks of headache, you can take rizatriptan or naproxen tablets. In order to reduce the headaches in future, you can take propranolol or topiramate tablets.

You should also avoid the headache triggers such as exposure to sunlight, loud noise or pollution. You should eat on time and sleep well. Avoid stress and tension.

If symptoms continue to recur despite treatment, a brain scan may be done to exclude other causes.

I hope my answer helps. Please get back if you have any follow up queries or if you require any additional information.

Wishing you good health,

Brief Answer:
Related to hormonal changes.

Detailed Answer:
Thank you for getting back.

Pregnancy is associated with hormonal changes, which have a direct bearing on migraine-related headaches. Therefore, headache frequency can increase in relation to pregnancy. Same explanation is given for exacerbation of headaches in relation to menstrual cycles in women with migraine.

It is unlikely to be due to brain tumor. This is because brain tumor-induced headaches would be constant and would not increase in relation to pregnancy. Other features of tumor-related headaches are: 1. Worsening of headaches in nights and early morning, 2. Vomiting with headaches, 3. Blurred vision or double vision in association with headache, 4. Some people would also have seizures or weakness of arm or leg.

I hope it clarifies.

Best wishes,

Dr Sudhir Kumar MD DM
