Hi! I Got The Urinalysis Result Yesterday As Part Of

Question: Hi! I got the Urinalysis result yesterday as part of the annual check-up and was told by the doctor that there was blood in the urine, for which the analysis should be repeated.

What's highlighted in the results is the following:

Occult Blood --- 1+
RBC --- 10-20 H

What is your opinion on this? I'm kind of concerned at the moment. Everything else was okay in the blood test results and urinalysis, including "no bacteria."

Hello dear and welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

I also reviewed the attached report.
There are several medical conditions that can cause red blood cells present in urine.

In my daily practice the most common cause is a bladder or kidney stone. They can be in the form of crystals or harder stones.
Other possible causes can be enlarged prostate or kidney disease.

I suggest you to do the following examinations to confirm the cause of your symptoms.
-An ultrasound examination
-Cystoscopy. Through a tiny camera it is possible to examine into your bladder urethra for disease.
-PSA blood level to check out the prostate

Treatment depends on the cause.
Meanwhile, I suggest you to avoid everything that can trigger the symptoms such as smoking.
I would also suggest to drink water and cranberry tea which can help in clearing out the urinary tract.

Dear Dr. Gurabhardi:

Thank you for your detailed response. I'm somewhat confused by your assessment of my microscopic hematuria, though. The main reason is because I have never had the slightest symptoms of bladder or kidney stone, UTI, etc. I haven't had any burning sensation or discomfort while urinating either. I have only felt a slight itching sensation inside the urethra every once in a while, and that's pretty much it.

In light of this confusion, shouldn't the doctor do a urine culture first? I mentioned in my first email that the doctor wants to repeat the test. I don't know if it is to overrule a false-positive, or to do a more accurate testing this time.

The PSA blood level was okay last year, but the doctor I saw this year didn't test me for that apparently. Anyway, I'm just confused because I don't have any symptoms (or history) that allude to what you wrote in your response.

Thank you for your questions.

Uroculture is usually recommended when we see the presence of an infection in the urinalysis.
This is presented by high WBC level or high bacteria level, which in your case are in normal range.

Kidney stones can also be asymptomatic.They can be detected only through ultrasound .
So, I would recommend doing an ultrasound for further evaluation.
I also recommend doing the PSA level, which should be done every year.

Dear Dr. Gurabhardi:

Thank you once again for your caring and detailed response. I didn't know that Kidney stones can also be asymptomatic. In this respect, one thing comes to mind: I take at least 500mg of Vitamin C (time released) daily. I may take up to 1.5 g daily when I feel I'm catching a cold. In this respect, I remember reading that Vitamin C intake may lead to kidney stones...Do you find any connection between the test results and this?

As for the PSA level, the doctor I saw this year said that she usually recommends it to males older than 50. I don't know the validity of this information.

At this point, I'm waiting for the lab results for the 2nd Urinalysis. I'll let you know what happens...

Vitamin C can influence in the formation of kidney stones.

Detailed Answer:
Hi again dear,

Vitamin C actually doubles the probability of kidney stones.
Kidney stones are often composed of calcium oxalate. When we take a big dose of vitamin C which usually is in the oxalate form, this may lead to the formation of the stones.

As per the PSA level I agree with your doctor that it should be done every year in men over 50 year.
But, based on your urinalysis I suggest you to repeat the examination to check out the prostate condition.
