Question: Hi, I had a cystoscopy last week. Now when I ejaculate it is painful and feels like the ejaculate is getting stuck towards the top of the urethra. Some comes out but I have to squeeze the rest out like a tube of toothpaste. My urinating is a little sore but doesn’t seem obstructed. What is going on? Thanks!

Thanks for the query.

Based on the facts that you have posted painful ejaculation and difficulty in passing urine after

Cystoscopy is most likely to be due to one of these two following two factors:

1) Inflammation of Urethra at the site of trivial injury caused during Cystoscopy.
2) Flimsy stricture (Narrowing ) of the urethra at the site of injury.

You should take prescription based antibiotics like Ciprofloxacin and anti-inflammatory medicine like Diclofenac twice daily for a week.

If the problem persists consult your Urologist for evaluation and further tests like Retrograde Cysto Urethrogram to confirm the diagnosis.

Ensure to drink more water.

Thank you so much for your reply. I wanted to give you some more information and to ask a follow up.

I had the cystoscopy last Thursday due to repeated UTIs and suspected stricture symptoms. My urologist stated that during the cysto everything looked “great” except for one small stricture that she “fixed” she did not say how she fixed it.

Since the cysto I have had burning and other unpleasant sensations during ruination, but those have been improving. During urination I will be doing fine and then suddenly I might feel a sharp brief pain in the urethra or almost a tugging sensation and then it goes away during the same urination. Generally, urine flow does not seem obstructed.

However, when I ejaculated several days ago, it felt like the ejaculate hit a clog inside the urethra about 3/4 of the way towards the tip of the urethra. I felt a lot of pressure and an odd pain at the site. Ejaculate did leave the body but struggled to do so. A good amount was also stuck in the urinary tract which I had to manually squeeze out.

My urologist didn’t seem to worried about any of this and said we could discuss it at my surgical follow up next Friday. However this is really worrying me.

Considering that my urologist said my urethra looked great, except for the small stricture that she repaired, what would you say is going on? Is it just post procedure inflammation? Could she have made things worse during the procedure by causing an injury?- I was bleeding a lot from the penis after the procedure and in a lot of pain. I wasn’t just urinating blood it was literally dripping out of the penis. I also urinated out a large blood clot and what looked like a large XXXXXXX colored piece of tissue as well. The nurse said that was normal but I have my doubts.

What do you think? I’m really freaking out (though urination seems to be improving, I dare not ejaculate again because that was terrifying)

Thanks for highlighting the facts in detail.

Whenever there is stricture (Narrowing ) of urethra observed during cystoscopy the concerned Urologist incise the hard stricture segment with knife in order to widen the narrowed segment what is called as Optical Internal Urethrotomy (OIU).

This causes injury and wound at the site of incision .The semen being thicker in consistency than urine does not pass out easily through narrowed segment of urethra before surgery and through inflamed and injured part of urethra after surgery.This normally takes 2 weeks to heal up completely .

I suggest you to avoid ejaculating either by masturbating or having sex till it heals up completely .

Do not worry it will get resolved after 2-3 weeks .
