Question: Hi,
I had an appendectomy about 4 weeks ago. The surgeon also repaired an umbilical hernia at the same time. Everything has been healing well until last night I woke up with stomach pains. I went to the bathroom and it felt a little better. I went again this morning I thought I had diaherria but it was actually blood. I just went again now and there was more blood. I was also lifting some very heavy things the night before and I don’t know if that caused something. Please let me know what you think this could be.

Thanks for your query.
Noted the history well and understood the concerns.
The commonest cause for blood per-rectum after 4 weeks of appendectomy that I can think is:
Bleeding internal piles/hemorrhoids.
Other causes can be Polyp, Diverticulitis, telengiectasia, low platelet and so on.
The bleeding from Appendectomy site after 4 weeks of surgery is almost unheard of although should be kelp in mind as healed wound can have a granuloma or so.
I would advise you the following in such a case:
Go to ER so that at least per-rectal examination can show for hemorrhoids or polyp.
May need Colonoscopy.
Blood tests will be vital and may indicate for any general cause.
Take care.
