Hi,
Thank you for the query. I can understand your concern. According to your statement, you have been suffering from gastroesophageal reflux disease (GERD) for many years with an interval of few years.
Barrett's esophagus is a complication of long lasting GERD. Barrett's esophagus may be resulted from the repeated injuries due to the acidic fluids that caused by chronic severe acid reflux or GERD. In such cases, the normal squamous epithelium lining of oesophagus may change into specialized columnar epithelium due to chronic severe acid reflux. Thus, Barrett's oesophagus is formed. The specialized columnar lining of Barrett's oesophagus is more resistant to repeated injuries from acid reflux than the normal squamous lining.
The process of developing Barrett's oesophagus starts at the junction of stomach and oesophageal lining. The common border of this junction is known as z line. Again, an irregular z line is characterized by <1 cm columnar tongues that extend proximal to the gastroesophageal, a finding that has been reported in about 10-15% of the population undergoing upper endoscopy. <1 cm z line doesn't appear to be at risk of any cancerous conditions like adenocarcinoma.
If Barrett's oesophagus is detected while undergoing endoscopy, then biopsies are also done because, Barrett's oesophagus may turn into any cancerous conditions in few cases. So, biopsy should be done to exclude any cancerous conditions in case of Barrett's oesophagus.
So, don't be worried at all. Nothing has confirmed yet without Barrett's oesophagus in your case. Your biopsy report may show non cancerous conditions as in case of Barrett's oesophagus such type of changes can find while undergoing endoscopy.
Take care. Hope I have answered your question. Let me know if I can assist you further.

Hi,
Thank you so much for joining again. Usually an irregular z line is found in case of Barrett's oesophagus. But finding irregular z line undergoing endoscopy doesn't confirm Barrett's oesophagus. But, it may be found most of cases of Barrett's oesophagus. Barrett's esophagus is confirmed by biopsies. And biopsy will show intestinal metaplasia that is the confirmation of Barrett's oesophagus.
An rregular z line should be <1 cm according to its definition. Larger than that may be called long segment.
In some cases, irregular z line may be found without having Barrett's oesophagus. Those z lines are normal variant. Usually 10-15 % of population may have z lines without having Barrett's oesophagus undergoing routine endoscopy. As you had no irregular z line on your first endoscopy report, so your irregular z line has been developed by injuries due to chronic severe acid reflux. For that reason, your irregular z line is linked to Barrett's oesophagus.
Take care. Hope I have answered your questions. Let me know if I can assist you further.
