Question: Hi, I had two blood test come back with high elevated levels of IGF-1 and IGFBP3. The dr did a hormone suppression test and said I Shpresa nicely. What can be other causes for this?

i meant to say he supressed nicely with the hormone suppression test.

High levels of circulating insulin-like growth factor-1 (IGF-1) and IGF-1 binding protein 3 (IGFBP-3) indicate a sustained overproduction of GH caused mostly by GH-secreting pituitary adenomas,/
Other potential causes for high IGF1 levels are low thyroid hormone levels. Liver disease. Uncontrolled diabetes mellitus , cardiovascular disease or intense exercises.
For the diagnosis of acromegalia a single random GH and IGF1 test is not enough to conclude the diagnosis because growth hormone or IGF1 are normally released in pulses and random levels vary widely.
It is also necessary to do an oral glucose tolerance test (OGTT) or hormone suppresion test.
If your GH and IGF1level doesn't drop to below 1 ng/mL during the OGTT, you have acromegaly.
Your GH level should respond to so much glucose by dropping, so if it doesn't, that tells the doctor that your body isn't listening to its own signals: The delicate feedback loop of the endocrine system has been disturbed.

would liver disease show up on routine labs?

If he has a pituitary lesion and high GH/IGF1levels,it can be a correlation.
If the tumor is <10mm it is called microadenoma.
Even a small microadenoma can produce pituitary hormones and cause symptoms.
Routine labs can show liver diseases.
