Hi, I Have 2 Years Old Son. Recently We Noticed
Although AST ALT in range but AST is touching upper border and APT being lower than AST, is it ok, because yellowness is there in body. Not only palm and feets but I can see body also kind of pale or yellow, therefore I was worried about this ratio reversal. Such ratio is reported every where associated liver serious diseases. How comforting are these reports with respect to that. And WBC being low is another worry for me, because it's being reported with serious disease like home marrow suppression, leukemia etc. Is neutropenia normal without these serious diseases too, like with viral fever or something. Because there are no sign of viral infection of any type. His breathe is giving kind of fruity smell , I have observed even after brushing, and when exhaling too. Can this be related. What test would you suggest ? And what possible causes for yellowish discoloration as per your opinion. He is fit and fine otherwise, no other symptoms appears to us, so this sudden rapid kind of discoloration we observed in last 6 days made me worried, and his low WBC also, whether he is prone to infection with such low neutrophils. As in his case his numbers are in severe neutropenia, which again is reported with serious diseases. Sorry for asking again, but please give detailed reply. Thanks and Regards.
Hi, doctor my queries are still unanswered, I am worried about present trend, and will come back after repeated test. But first you said that ratio is not an issue as AST ALT values are in their individual range, but here in your reply second time, you have said it's warning sign. Both are kind of opposite, sorry for asking that, but if you find this report worrisome for any reason please tell in details so that actual worry can be found out. I have asked about infection prone due to severe neutropenia, and any precautions we might need to take, is unanswered? and is it any way connected with possibility of leukemia or any serious disease. Have you seen any cases with such neutropenia due to other reasons too. Which blood test we need to go for for clearing out doubts@
Last query , I afraid for leukemia and liver cirrocis, how safe we are from your point of view with present report only. Thanks and Regards.

Detailed Answer:
Hi and thank you for query,
Answers are-
1)Low WBC count with elevated AST/ALT ratio is suggestive of viral infection.
2)Neutropenia( decrease in neutrophil) is indicàtor of viral infection.
3)Lal path lab report is not opening. Kindly mail it at YYYY@YYYY or reattach it.
As off now after going through the photo attached( Yellowish pale skin could be seen).
The Golden rule of medical science is common thing first. Rare things do occur but they occur rarely. So I would suggest you to-
1) Repeat test at another lab.In covid phase most of the XXXXXXX consultants are not available and tests are done by junior staff so repeat the tests at another lab for a cross check.
2)Hepatitis panel test should be done. Very likely he is having Hepatitis A which is one of the most common infection in XXXXXXX Its viral in nature and settle back in 14 days.
Precaution-
Definitely...Neutrophils are fighter cells of body. Any attack on body is first retaliated with Neutrophils.
As off now covid 19 a highly contagious infection is there and first line fighter cells of the kid are low so he is at higher risk of infection compared to normal individual.
If possible buy N 95 respirator for him and ask him to wear it properly(As he is at risk of catching infection). Strict home quarantine for kid till neutrophils are back to normal again.
Suggestion-
-Repeat the test again and hepatitis panel test should be done along with stool and. urine Routine and microscopy test.
-Don't Google to much.
-Follow up with reports.
-Diet-Simple carbohydrate based diet(60%) fruits vegetables juices.
20% Protein( lentils soya etc)
20% fats.
Revert back with reports.
-No leukemia etc at present.
Don't worry too much.
-Mail me the reports(as the answer is based on photo of hand and details written by you)
waiting.
Regards!
Please go through detailed answer dear.

Detailed Answer:
Hi dear,
I am sorry the report of CBC were not opening on my laptop in previous answer.
I don't think it's related to liver. His TLC is also low. This means that total number of fighter cells in body are low.
These fighter cells consist of immediately acting group called Neutrophils and Back up soldier called Lymphocytes. His absolute neutrophil count is very low and Lymphocyte too are very low.
Patient is totally asymptomatic.
I am sorry but I would suggest you to contact a Hematologist for the same.
A secondary consultation should be taken from a Oncologist.
ALT AST does not matter that much but first priority should be to rule out bone marrow suppression (Bone marrow produces fighter cells).
Suggestion-
Consult a Oncologist and Hematologist.
While taking him to hospital try that he wears N 95(even if you have to force him..He is highly suceptible to infection.
Best way is to consult a good Hematologist and Oncologist online with these reports.
Prons:Cons ratio of taking kid to hospital right now should be seen.
I don't think this crisis is going to end soon.
Waiting is not a good option.
Even you can buy a hazmat suit online and once the patient had visited oncologist just discard the suit.
I would suggest you to-
1) Repeat CBC-Path lab can collect sample from home.Ask the collection boy to use hand sanitizer and while taking sample put N 95 on child's face(1-2minutes).
If reports are same consult oncologist asap.
Don't panic but consultation is required.
Follow up after that.
Let's hope for the best sir.
Regards.
