Question: Hi, I have been having a lot of trouble breathing and was hoping to get some help on what the issue could be. It first happened two weeks ago I woke up in the middle of the night coughing as if I was choking on something. When I woke up in the morning my throat was agitated, I had a dry cough, and some mild shortness of breath. I went to the urgent care and they just told me I had an upper respiratory infection and gave me medicine for the cough. I didn't have any issues later that day or the next day. But in the evening two days later I started to have trouble breathing again. It gradually got worse to the point where I started to panic and went to the er. They did a chest X-ray, which they said came back showing nothing and did a breathing treatment which didn't help. They still prescribed my an albuterol inhaler which also didn't help. I went back later, after continuing to have trouble breathing and worsening anxiety. He prescribed me lorazepam, which helped a little bit, but I still woke up with anxiety and shortness of breath. I saw a physician a few days later and she prescribed me Effexor xr for the anxiety, assuming the shortness of breath was from that. The anxiety remained for a while, but since Monday has totally gone away. The shortness of breath has remained though. I saw another doctor who put me on lisinopril cause my blood pressure was high. That didn't help the breathing. I have also tried Nexium, as I had some symptoms of acid reflux and that hasn't helped either. Since yesterday I've noticed it being increasingly more difficult to breathe particularly through my nose. It feels very congested, however there isn't any mucus or anything when I cough or blow my nose.

I can understand your concern and I read very carefully your history.

So according to the history you provide you have already exclude some very common causes of your cough.

In my opinion this might be a cough variant bronchial asthma triggered by an upper respiratory tract infection (viral or bacterial )

If you were my patient I would recommend you to do some changes in your current treatment as follows:

Start a topical steroid Flixonase spray three times per day in each nose.

Start also an inhaler such as Budesonide twice for up to one month.
Meanwhile continue Nexium instead because even a silent gastric reflux might deteriorate the breathlessness.

Please follow the instructions and you will soon feel better.

However please first discuss with your doctor for the above and I am sure that he or she will take them in consideration.
