Question: Hi,

I have had tearing or cramping type pains across both sides of my lower abdomen area for about 2 or 3 months now. It is very random as to when it happens though. This past Monday I had a colonoscopy due to the listed symptoms as well as having high sharp rectal pain and blood in my stools at times. They found some internal hemorrhoids and banded them. Well I have still been having the cramping or tearing type pains in my sides and lower abdomen. I had my bloodwork ran Monday evening and everything looked normal and was in range. They also ran a CT scan on my lower abdomen that showed it looked normal with no infections or tumors. I do get gas fairly often and my stomach feels bloated much more than normal in the last few weeks. Both the ER doctor and the gastroenterologist had no answers for me and seemed baffled by my symptoms. Can this be due to a gluten or lactose allergy possibly? Should I try taking a probiotic pill? I am at a loss and would like some answers. I am not sure if this is relevant, but I had a fusion on my C5, C6, and C7 on July 18th and have been recovering well.

Thank you,

Brief Answer:
Colonoscopy should be followed by MRI whole abdomen.

Detailed Answer:
Hello again and thanks for choosing "Ask a Doctor" service for your query,
Have seen your details and I appreciate your concerns,
Very likely it could be a sprain or muscular inflammation as colonoscopy and other tests are normal.
So we would go out of the box now
I Would suggest you an MRI of the complete abdomen.
Principle of MRI is based the fact that it splits the water inside every organ ligament and muscle tissue. If there is any part where blood supply is compromised due to any reason may be wear or tear of muscle ligament etc the blood supply to that area would be compromised and hence the water supply would be compromised. This would form a fade image when brought in presence of high magnetic field of MRI.
Discuss this with your doctor and get an MRI done.
Hope I was helpful.
Follow ups are welcome
Thanks you.

Do you think it is possible that it could be IBS or Colitis? Or even a gluten or lactose issue possibly? There was no pains when the doctor pressed down across my lower stomach at all. He did do an ultrasound of my middle and right lower abdomen areas and said there was no indication of a tumor, infection, or appendicitis. I was going to keep a daily log of all my foods I eat and try to stick to high fiber foods.

Detailed Answer:
Hello and thank you for follow up,
As your pain is limited exclusively to the right and left lower quadrant lets first discuss the anatomy of those parts before moving on to disease etc.

Right iliac fossa contains-
Appendix and caecum while Left iliac fossa contain descending and sigmoid colon.

As far as gluten and lactose allergy is concerned they develop overtime and not in just a short span of time.Secondly removing them from your diet for even 3 days would result in symptoms end.

Definitely yes i would suggest you to take probiotic good quality like VSL#3 twice daily for minimum of 21 days however i don't personally think that the tearing pain is due to that.
AS per as CT scan is concerned its nothing but X rays rotated through 360 degrees and slices taken along various angles(Not of much use in case of abdomen).Ultrasound is just a baseline test.

There was no pains when the doctor pressed down across my lower stomach at all-This means that inflammation and tenderness is not there making issues more complex. Therefore i would suggest you to go for an MRI to rule out if blood supply and functioning of each and every individual organ etc is fine,Coilitis means inflammation and would be reflected in colonoscopy and CT.
As far as IBS is concerned its not a disease rather a group of symptoms. When a person is symptomatic and despite of all possible tests related to gastroenterology nothing could be determined patient is termed to have IBS.
To be declared as IBS complete abdominal testing A to Z should be done.
Secondly IBS pain is usually spasmodic and not tearing in nature.
As your CT scan is normal(no diverticulitis) tearing nature of pain could be suggestive of an internal ligamental or muscular injury(if you are obese then chances are even more).

In case you don't want to go for MRI right now take over the counter medicine Ibuprofen thrice daily with a probiotic for 3 day ans respond back to me.If relieved its muscular.

Hope i was clear and easy to understand,
Get well soon,
Thank you!

I am about 6ft2 and 220lbs when last weighed at the doctor office. A good portion of my weight is muscle mass. I read on my colonoscopy post op it mentioned that cramping and bloating could be a side effect after the procedure. How long can that last usually? It has been a week now. He banded two internal hemorroids. The instructions also told me to take a stool softener for two weeks. I have been taking 5mg dulcolax once a day for a week now. When I get this cramping feelings in my abdomen I have been taking pepto bismol or pepcid complete at times. Is that a bad idea? I am not sure if the Dulcolax could be effecting me this way.

Hello and thanks for follow up,
Answers are -
At the most a day or two unless the procedure is done by a complete new bee.
No not at all a bad idea.Banding had been done and the part is still injured and is healing. If hard stools would pass through that area it would cause erosion and bleeding therefore a stool softner or laxative should be taken. But Dulcolex is way old take 10 ml of lactulose(Over the counter) its much better.
Switch over to lactulose from Dulcolex and tell me how you feel in a day or two.
Hope i was helpful,
Thank you.
