Question: Hi, I was involved in a car accident yesterday, I have pain in my back. I have L1 compression fracture with mildly retropulsed fragment. What can be done for me, I mean using the safest possible solution. I also have osteoporosis. Attached CT scans. Sincerely, Kal

I have read your query and here is my advice.

Mild to moderate L1 compression fractures may be treated conservatively (i.e. bed rest and symptomatic treatment for 6 weeks) if there is no instability and no neurological deficit.

If there is instability and significant spinal cord stenosis, the best treatment is surgical stabilization and decompression.

Since there is osteoporosis and no neurological signs I think you could be treated conservatively, however, it is necessary to see the CT scan in order to give you a comprehensive answer.

I examined the CT scan images and, in my opinion, your case can be treated conservatively with bed rest for 6 weeks.

This because the fracture is less than 30% and the retropulsed fragment is small.

There seems to be no fracture of posterior elements, so, the stability of the column is not affected severely.

Hi, Thank you very much for your kind and much appreciated help, I'm staying currently in a local hospital in my county and the doctor here is pushing for a surgery which I don't prefer as their skill here isn't that good!
