Question: Hi, I'm a 39 year old, 140lb woman with no family history of autoimmune diseases or Hashimoto disease in either side of the family. My TSH was high in my late 20s (age 28, TSH 18).I did not take any medication and it normalized by my early 30 (32-33) with the use of kelp iodine supplement. It stayed normal until now (39), I was checking it yearly via blood test. T3, T4 and Free.T4 always remained within the normal range, even when my TSH was 18 (that was my highest at age 28). So now I'm 39, my TSH was normal in August 2019 but went up to 5.66 by April 2020. T3, T4, Free T4 still in normal range. I started to take 75 mcg synthroid daily 3,5 months ago and my TSH dropped to 0.01, Free T4 elevated to 2.0. My doctor suggested to reduce the med to 37.5 mcg (take 1/2 tablet instead of 1) daily. However, I belive it's still way too high dose considering how fast my TSH dropped, that my T3, T4 levels were always normal (except now, when I'm overdosing) and that no family history of Hashimoto. So I decided to ask for a second opinion. What dosage, if any, should I take. I'm trying to conceive so thyroid function is extremely important right now. FYI: I take CoQ10 and a general 1/day multi-vitamin supplement, no other prescription meds, no HBP, no high sugar. I take my vitamins at night to avoid interference with synthroid absorption.

I have read your notes. Ideally the TSH level should be maintained in the lower half of normal range when you are trying to conceive.
My opinion is that, as of now continue taking 37.5 mcg of Synthyroid. After taking this dose for three weeks, repeat the thyroid function tests again. Accordingly synthyroid dose can be adjusted if required.
What is your BMI? To improve your chances of conception you need to maintain normal BMI, stop smoking if you are a smoker, and have sexual intercourse 3-4 times in a week.
Have you been tested for thyroid antibodies? If not done, please do it.

Thank you for your detailed reply. My BMI is within normal range, I'm 5'3" (160cm) and anybody is not an issue, that's why I'm having trouble accepting my hypothyroidism diagnosis.

Shouldn't I take minimal synthroid, like 12.5 mcg or 25 mcg until my body uses up the excess free T4 (for like 1-2 weeks or something) and then take the 37.5 and try to see how my body reacts to the lower dose?

I mean really, if i wouldn't try to get pregnant, I'd try to normalize my TSH without hormones... Everything but my TSH level was within normal range.

I feel like my body needs a minimal adjustment, not a heavy hormone replacement as if my glands were not producing any... But I could be totally wrong, of course.
