Question: Hi I’m a 43 year old female. For the past day or so I have been feeling a spasm like pain come and go located towards to lower right abdomen low down towards my hip going across towards my vagina. It began on a long car ride don’t know if I was sitting in an awkward position or what. I’ve had this pain before over the summer my gynecologist arranged an ultrasound and all came back clear what could this be should I be concerned

I have gone through your history and understand your concern.

The right side abdominal pain could be due to so many causes like a kidney stone,appendicitis and ovarian cyst. If the pain is severe, I will insist to visit an ER immediately. If not, you should drink a lots of warm water and soft diet. Take tylenol or ibuprofen for pain relief. And if the pain is persistent, it is better to go for CT scan of abdomen to rule out the possible cause of your symptoms since your last ultrasound scan came normal.

Would the pain of appendicitis come and go over months the pain is not severe but sporadic in nature I have an appointment with my gyn next month is it safe to wait I assume ruptured cyst appendicitis would cause severe constant pain other symptoms ...

Yes,you are right if you have persistent severe pain it could be due to rupture cyst or appendix and may need immediate intervention.
But in your case it is dull aching pain, not so severe pain,it is due to some chronic disease.
Yes appendicular pain can come and go if chronic in nature.

Accordingly I think you can wait and watch.

Yes if pelvic muscle is stiff it may cause lower abdominal pain or spasm but it is usually on lower abdomen.
