Question: Hi. I'm a 54 yr old female with chronic itching at the top of the buttocks' crack. Sometimes it also burns. I've had it over two years. I've tried Lotramin Ultra twice a day for two weeks, thinking it might be something similar to jock itch, but it didn't do anything. It would relieve the itching for a moment, but it perhaps has only gotten a little worse. Do I need to see a doctor or is there something else I can try first?

I understand your concern. The first assumption for this rash which has been lasting for two years does seem to be a fungal infection, however since Lotrimin did not help it needs further evaluation. It could be chronic dermatitis, psoriasis (assuming you have a history and the rash is scaly and also located elsewhere), or it could be a recurring sacral herpes (painful vesicles). Hard to say, however it would be very helpful if you attached a picture of the rash so I can have a better idea.
