Question: Hi,
My mother has been suffering from uneasiness and loss of breath for few days. On consultation with the doctor she suggested for Holter test.. The reports shows the decreased pulse rate most of the times.. Now doctor is recommending pace maker but her sugar levels are high and hence the doctor had recommended us to wait till her sugar levels become normal. Please suggest if we should wait . Also, find the attached test report.

Hello

After going through the medical details provided by you I can understand your concern and I would like to tell you that probably the doctor has seen some significant rhythm abnormality in the HOLTER report for which he has advised pacemaker.

As her sugar levels are high so yes it’s very important to wait till her sugar levels comes within normal range because high sugar levels make the patient susceptible for infection there by causing pacemaker pocket infection and sometimes septicaemia.

If by the time she experiences significant pulse drop or symptoms likes syncope then she needs hospitalisation.

What range is considered as significant drop in pulse, as her pulse rate sometime drop to 44-42 but after a whiler when we monitor again it comes to 76, is this a matter of concern? Also, her pulse rate are low early in the morning and just before when she go to sleep her pulse rate are 44- 46.. Also, when she is empty stomach she has low pulse rate, is hunger related to pulse rate by any chance?

Pulse rate also has diurnal variations and it goes down when the individual is at rest or during sleep.
The pulse rate should not go anytime less than 40bpm.

No hunger doesn’t affect heart rate significantly.
