Her diabetes is very much uncontrolled. This much high blood sugar can be a sign of infection also. Urine acetone should be tested to rule out keto acidosis. Better to shift her to ER (casualty). Intravenous Fluids and electrolytes also will be required in such case.
Urine routine and blood routine should be done to rule out infection. In case of positive signs of infection antibiotics should be started. Insulin should be increased also. But at this much high blood sugar Plain insulin (short acting) is advised. This can be better managed in hospital setting. Once sugar goes controlled then can shift to insulin pen (long/intermediate acting).
