Hi Sir. I Am Taking Pioz MF 15 Since February

Question: Hi Sir. I am taking Pioz MF 15 since february 2017 and my sugar level is in control now. As per latest test reports my fasting sugar level was 104. currently this medicine is not available in medicine stores as suppy is restricted for this medicine. Please suggest some alternate medicine for Pioz MF 15.

Pioz is one of the meds that can be used for diabetes but because it is associated with heart failure (it can cause or worsen heart failure), persons are not prescribing it as much as before.
Up to 8% of persons who use it can get heart failure.

Alternatives are : metformin- this is one of the older meds but one of the best, it lowers blood sugar but does not push it too low to cause hypoglycemia. It also lowers to body's resistance to its own insulin, this means that the insulin in your body works better.

No weight gain

(2)there is a family of meds called DPP inhibitors eg sitagliptin, saxagliptin, linaglipin- also work well, no weight gain, no too low blood sugars

(3)There is another family of meds of drugs called GLP 1 agonists eg exenatide, liraglutide.
In addition to lowering the blood sugar, these meds also help weight loss if weight is an issue

(4)sulphonyureas are also available but they cause weight gain and low blood sugar eg amaryl, diamicron etc

(5)insulin- there are a number of different types of insulins that can be used

Hi Sir. Please suggest any good brand name for metformin as I want to try it first. Also please suggest what will be dosage for metformin which will be equivalent for Pioz MF 15.

Metformin is a good choice, metformin is the actual drug name. There are a number of different brands but the active ingredient is the same.
Some of the more common brands are glucophage, riomet, diabex, obimet

The dose that you can start with would be metformin 500 mg once daily. You should consider the long acting form of metformin which is metformin xr. It can be increased to 2 g daily if needed

This type of metformin is released slowly in the body and this means that you only need to take it once daily and it also means that it is less likely to cause side effects (since only small amounts released at a time)

It can be taken with meals to reduce any effect on your stomach. It can be taken at any point during the day.
Choose a time that you would most likely remember to take it. If you think that you would most likely remember after breakfast , then use it at that time.

If you think that you would be more likely to remember around lunch or supper time, then you can use it then.

Typically, you would start noticing a decrease in 24 to 48 hours, the maximum effect would be seen after 3 days of taking the medication.

If at day 3, your blood sugars are not near the goal then it may mean that the dose would need to be increased to you reach the strength that would work best for you
