I think I need urgent suggestion/help/support for my brother's situation.
He was highly stressed out since last few months (before XXXXXXX '20) due to personal feelings of exploitation in the family. The stress was also increased due to the lockdown and Covid 19 situations. However, he was involved in high complex skilled projects and was performing exceptionally well in it at individual level.
On 1st XXXXXXX '20, he unusually woke up very early and was highly anxious. He was also feeling difficulty to speak much in a panicking behaviour. The situation around him felt highly abnormal and suspicious. He faced similar issue the next morning as well when he could'nt sleep well and woke up very early and wanted to talk with all the family members instantaneously.
After the incident, he experienced the following issues for the next 2-3 days:
- Minor change in behaviour
- Restlessness and lack of sleep
- Change in taste
- Headache and excessive sweating
- Weakness
He seemed to be feeling better after consultation, good sleep and consumption of the following medicines for 1 month (I.e. till 1st July '20):
1. Petril MD 0.25 for 5days
2. Tolperitas SR 450 (1 tab for 10days)
3. Tab Prothiaden M 75(1 tab daily)
4. Tab Palmiges (1 daily)
5. Tab Zolsoma 5 (1tab daily)
6. Syrup Cobatus (2tsp daily)
7. Inj Rejunex forte ( Once a week)
Few observations before 1st XXXXXXX '20:
1. He was feeling highly stressed since last few months due to professional and personal issues.
2. He had a mild headache and body pain whenever I woke up in the morning.
3. He was highly analytical in studying the root causes of different issues.
Few observations/changes after the incident on 1st XXXXXXX '20:
1. He was facing difficulty in processing complex statements.
2. The very recent memories seemed to be affected.
3. His personality seems to be changed a bit with changed behaviour and moods.
4. He seems to remember and talk about the minute incidents of the very previous past.
The behavior and moods have turned very violent in the last 1 month with multiple instances of using the word 'suicide'. He is also getting very very irritated on minute things like switching off the lights or changing the TV channels.
He has also socially withdrawn himself from all family and friends.
Whenever there is any interaction, it leads to furious anxious reactions from his side.
He has been sleeping either for a very long time or is facing difficulties sleeping when he gets irritated for any minute thing.
I need help to support him from furious reactions and suicidal thoughts.
He is reluctant to talk to anybody and is almost silent for the last few days except for instances where he feels that everybody is plotting against him. We have been trying to improve the guinine concerns in his situation related to job, social circle, etc but it's taking it's natural time to get normal.
He does not want to talk to any psychologist or doctor nor does he seem to be improving in the last 3months now.
I am extremely worried for him. It will be great if you can please suggest how can I help him.
In recent times he has been very aggressive and adamant on suicide even keeping sharp objects with him when he is very anxious.
As suggested by him, we have tried to provide him the maximum personal space and prevent from triggering factors.
We have also tried to take short trips and food outside tonxhange the mood. It has helped for time being but doesn't seem to help in the long run.
I am unable to figure out the causes and am very worried about him. It will be very helpful if you can please suggest.

First of all I like to inform you that the available details clearly indicate possibility of psychiatric disorder (most likely depression with anxiety) and I do not think without proper treatment it is possible to get him on the track. With turning of events I feel he is also developing psychotic features such as Paranoia and refusal to seek any medical advice.
It appears that due to lack of insight about his own illness he is making irrational demands of such as maximum personal space and I feel leaving the patient alone gives more room for self harming behavior.
The medicines, if taking them regularly, needs complete change as no improvement after three months of use indicates that current medications are not working.
In case you are not able to convince him for medical assistance judicial/administrative services (atleast a rank of SDM or above) can be approached so he can be detained for suitable centre for treatment.

I like to clarify that most of psychiatric disorders (including case under our consideration) often require longer duration of treatment and it is recommended to continue atleast six months after symptom resolution.
The medication and counseling are often helpful and should be prescribed when required.
