
Hi Doc My Question Is For A Urologist I Have

My question is for a urologist I have uploaded two images. My concern is the shape and size of my left testicle.
1) it hangs much lower.
2) when it is loose I can’t see any swelling and the testicle size feels the same.
3) when testicle are closer to my body.. I can see the left on hangs lower. I have attached two images for you to look at does the left testicle look normal to do. I am sure you can see from the shape and size. My worries were a hydrocele but I’m guessing if this was the case it would be in the front part of the testicle and wouldn’t be found at the bottom where the epidiymis tail is you know that pointy bit on the bottom?
I did get checked by a doctor last year and regularly look out for any changes.

Based on the facts and photographs that you have posted I would like to answer your queries as detailed below
1) In almost every individual left testicle is at lower level than its counterpart .It is normal and not to be worried about .
2) Size of the both the testicles are not identical .It is only to be concerned only when one is comparatively noticeably larger than other .As per your photographs both the testicles are normal .
3) As regards Hydrocele it is very difficult to differentiate minimal hydrocele from normal testicle .It can be confirmed only by clinical examination in person and by doing Ultrasound Scanning Of Scrotum.As such minimal hydrocele has no significance .
Consult qualified General surgeon for clinical examination to confirm the same .It will help to allay your anxiety .

I have a few follow up questions
I have uploaded two photographs of my left testicle does this still look normal it is from behind the testicle?
One I have left a green mark on is it normal for the testicle to have such a anatomy?
I had a examination from my GP somewhat last year and he said the testes is normal
Just thinking outside the box
1] Does hydrocele effect fertility?
My understanding of a hydrocele is - that it forms and can dilute sperm as water is in the testicle?
2) Is hydrocele in contact with the testicle or would it be in a membrane before and have no contact with testicle or other organs.
3) do spermatocele or hydrocele effect fertility.
4) is it normal to have some water surrounding the testicle? I’m guessing the amount differs in men.

6) green arrow image - is that the epidydimis tail? And normal anatomy

It is difficult to comment about epididymis or any abnormality either in testis or epididymis on viewing photographs .
It is the clinical examination by a qualified General surgeon or Urologist who can comment about it .
Answers to your questions are detailed below
1)Hydrocele is collection of fluid outside testicles and it never comes in contact with sperms hence there is no question of diluting sperms .
2) The testicle is covered a very thin membranes called as Tunica Vaginalis which has two layers The inner layers secrete a watery and oily fluid that gets absorbed by outer layer .This is a continuous process and keeps the testicle freely XXXXXXX in side the sac.However whenever there is infection in scrotum (Testis) inner layer secrets more fluid than what is absorbed by outer layer, disturbing the natural balance of this process of secretion and absorption and this extra fluid gets accumulated between these two layers that cover testicle that is what is called as Hydrocele
3) Spermatocele or Hydrocele do not affect fertility.
4) Testicle is always immersed (Covered)in a small quantity of fluid which I have explained above .
5) The normal anatomy either Testis or Epididymis can be commented only on palpation of the testicle in person and can not be opined on photograph.

Final question
Can a hydrocele be found on the bottom part of the testicle where the heads tail is? Or does it usually arise from the top.
I don’t need any check up ur advice and reassurance is enough I’m a young man need to enjoy my life!
