Brief Answer:
Need to do Prostate Biopsy to rule out cancer of Prostate.

Detailed Answer:
Hello
Thanks for query .

Based on the facts that you have posted PSA readings of your husband is raised as reported in two consecutive blood tests over a interval of 6 weeks .

You have not mentioned age of your husband .

I shall be happy if you can post me more details to help me in making precise comments
1) Has the Urologist done Digital Rectal Examination and opined about presence of any hard nodule on surface of prostate ?

As per normal protocol raised PSA above normal value of 4 is always suspected to be due to Cancer of Prostate unless proved otherwise by doing Ultrasound Guided Multi Core Biopsy of Prostate.

Please visit your Urologist again and discuss the same with him regarding doing prostate biopsy..

Hope I have answered your query ,please feel free to ask if you have more questions,I shall be happy to help you
Thanks and Regards.

Detailed Answer:
Hello
Thanks for follow up .

Since you have not asked specifically about the question which you have asked now in follow up in your earlier query I have not opined about whether person can have cancer of other organ after having cancer of one organ very recently.

It is not uncommon to get cancer of multiple organs one after the other .

Thanks and Regards.

Urimax does not have action of lowering PSA

Detailed Answer:
Hello
Thanks for follow up .

Urimax is an alpha adrenergic receptor blocker and it relaxes smooth muscles in prostate and bladder neck and helps to improve urine flow .It has no action of any nature on secretion of PSA by Prostate hence PSA is not lowered even after taking Urimax.

Brief Answer:
Get MRI done

Detailed Answer:
Hello
Thanks for follow up.

PSA is also raised in following conditions
1) Benign Enlargement of Prostate with secondary UTI
2) Acute prostatitis

In such conditions we prescribe antibiotics and Dutasteride for a week or two These drug reduce PSA as the congestion of prostate and infection subsides ,However when ever the PSA reading are high ( More than normal value of 4) as detected on two successive blood tests done at the interval of month or two it is the international protocol to be suspected to be due to Cancer of Prostate which demands either CT guided or MRI guided biopsy of Prostate which is the most confirmative tests to confirm the diagnosis of Cancer of Prostate so that further treatment can be planned out,

Hope this will resolve your doubts .

Thanks and Regards.

MRI detects the confinement of cancer

Detailed Answer:
Hello
Thanks for follow up
Antibiotics and Dutasteride are prescribed if the Dr who has carried out examination suspects it to be Benign Enlargement of Prostate

MRI is noninvasive test and helpful to detect cancer of prostate and whether it is confined to prostate or spread outside If it is positive then biopsy is advised.
whereas biopsy is invasive process and slightly painful

Thanks and Regards

Brief Answer:
Taking whisky does not increase Urea and Creatinine.

Detailed Answer:
Hello
Thanks for writing again ,

His creatinine and Urea is raised due to burden over a single kidney (His one kidney being removed earlier ) or due to infection which needs to be investigated .

Is he diabetic?

Taking whisky does not raise the creatinine or urea levels in blood .

Brief Answer:
Raised level of Urea and Creatinine indicate deranged kidney function .

Detailed Answer:
hello
Thanks for follow up .

Urea and Creatinine are end products of protein and muscle metabolism of the body and excreted through urine, however level of these products in blood is raised whenever kidney is unable to excrete these products in urine either due to paranchymal disease of the kidney or some other pathology.

In your husbands case the kidney function is compromised mostly due to over load over the remaining kidney after removal of other kidney.

The present readings of urea and creatinine of your husband are not alarming and in acceptable range ,However he would need to observe it and monitor the same at frequent intervals .

Thanks and Regards.

Plain MRI can be done even if serum creatinine is raised

Detailed Answer:
Hello
Thanks for following up .

Raised serum creatinine levels suggest kidney disease which is a contraindication for giving contrast radioactive medium before doing C T or MRI with contrast. However, plain MRI can be performed which also gives details of the inner structure of the Prostate.

Discuss with Dr Nadar .

Brief Answer:
Get Total PSA done

Detailed Answer:
Hello
Thanks for follow up .

There is lot of debate and controversy over usefulness of PSA to decide over need for Prostate Biopsy. However the ratio of Free to Total PSA gives us a guide line to take a call for the same This decision making deffer from one Urologist to other and it depends upon the individual experience and perception .However you need to get both free and Total PSA done simultaneously..

I would suggest you to get his MRI done and let your Urologist decide about the need for prostate Biopsy.

Thanks and Regards.

PSA can rise rapidly if there is active infection in BPH.

Detailed Answer:
Hello

PSA does rise rapidly if there is an active infection .It can also rise if there is cancer of Prostate

Please note that raising PSA is a screening guideline to decide need for biopsy ,The final diagnosis is confirmed only on biopsy .
