Question: Hi doctor, I am an old patient of yours with issue of anxiety and sleeplessness. I had severe coughing for two months and not getting cured either by ENT or Physician. I then started taking Nexito 5 in the morning and coughing disappeared.
I was consulting another psychiatrist for my sleep issues and he told me to stop Nexito 5 and take Zapiz 0.25 and Mirtaz 7.5 at night.
I am taking this for almost 15 days but the sleep comes very late around 1 AM. Coughing has also restarted. I referred side effects of Zapiz 0.25 and found to be addictive.
Finally I decided to come back to you. Please advise.

Cough responded to Nexito most likely because anxiety was the reason of cough. A lot of individuals develop mild cough or throat clearing like symptoms during anxiety.

Zapiz 0.25 mg is not addictive if you take the drug regularly in prescribed doses for short time. Mirtaz 7.5 mg is safe and you can increase the dose of the drug to 15 mg and this should help in sleep.

You can start Nexito 5 mg again in morning and the drug can be taken with Mirtaz and also with Zapiz.

No need to get worried.

I had sent follow up questions twice but unfortunately the app failed to show up. I called the admin also and complained. He told me to send the follow up question using website and not app.
I have following doubts:
1. Short time is how short? 15 days, 21 days, 30 days? 1 mg app mentions clearly that Zapiz 0.25 is habit forming and you are asking me to continue.
2. I had a problem of breathlessness in September 2013 and my physician at that time started Nexito 10, which cured me. I already had suicidal tendencies. I came to know for the first time about anxiety.
3. Later in 2019, I had problem of not getting sleep and a psychiatrist on DocsApp advised me to stop Nexito 5 (He only made me reduce the dosage of Nexito 10 to 5 earlier). He advised me to take Zapiz 0.25 and Mirtaz 7.5.
4. I also had severe coughing since December 2019 and I tried taking support of ENT surgeon and also a physician but no improvement was observed. Coughing continued till March 2020 and simultaneously corona also popped up, so I was afraid that my allergy & anxiety may be taken by people as corona.
5. I started taking Nexito 5 and second day only my coughing stopped. But as advised by another psychiatrist, I stopped Nexito after only one day and started Zapiz 0.25 and Mirtaz 7.5.
6. Now for more than 15 days, I am taking both Zapiz and Mirtaz. zapiz being habit forming, can I stop that and take only Nexito 5 in the morning and Mirtaz 7.5 at night? My sleep starts around 1 AM and eyes open around 10 AM. What do I do to get sleep latest by 12 night?

1) 1 mg is not doctor. Most of articles on that site are written by not so qualified professionals. If taken under proper supervision for less than 4 weeks then the drug is safe. I have a lot of patients who are on this drug since long and they are doing it.

2) You can stop Zapiz if you have persistent doubts about this drug. Even Mirtaz will help in sleep. No need to get worried for that. Nexito will control the anxiety and its exacerbations.

3) For getting sleep earlier than usual time you can take Zapiz in dose of 0.25-0.5 mg for short time. This will help in sleep onset and with time your sleep cycle will get normalised. This can be done and this will not increase risk of dependence.
