Question: Hi doctor My tongue has patches and feels like little pain Sometimes bleeding as well Please advise I do smoke some times and drink some times Thank you

The patch over the tongue can be a leukoplakia if it is white in color or if the patch is red it can be a depapillation of the tongue. Kindly describe the appearance of the patch present over the tongue in detail or upload the images if the same.
The bleeding from the tongue can be secondary to any injured inflamed papilla.

I have gone through the images. As per the images, it looks like geographic tongue and coated tongue. Geographic tongue usually occurs secondary to stress or nutritional deficiency. The coated tongue occurs secondary to improper cleaning of the tongue surface,use of alcohol and smoking. Coated tongue appears as a white patch and geographic tongue appears as red patch. So consult an oral physician and get it evaluated. If i am your treating doctor, i would have suggested you to take multivitamins and use tongue scraper to clean the tongue and stop the habit of smoking and drinking. Reduce your stress level.
