Question: Hi doctor ,
My wife is pregnant . She had started last period on 05 XXXXXXX 2021 and she missed period on 05 feb . Her home pregnancy test had two line i.e. positive i.e. pregnancy confirm on 10 feb . Then we approach doctor on 13 feb , Doctor said to take blood , urine tests and sonography ( all kind of report attached) . Reports came and doctor said there is pregnancy and also a mass cyst present , as shown in attached report . Doctor recommended 4 medicine and called after a month for check up :-
1. TAB - RAVER IT -rabeprazole sodium , itopride hydrochloride , morning empty stomach ,
2.TAB - GUDDAY - doxylamine succinate , folic acid , pyridoxine HCL , before 15min of lunch
3. TAB POLAR9 - L-methylpholate , methylcobalamin , pyridoxal 5 phosphate , in night after food ,
4. SYP VILIZYME - papain , vit b complex , fungal diatase , 2 spoon subah sham )
and she started these medicine from 14 feb .

she feels pain in abdomen , nausea , headache , body pain . In report less hgb , pcv , mcv , mch , mchc , eosinophils and more neutrophils than range .

She didn't started folic acid and multivitamin until 14 feb as pregnancy was sudden and unplanned .
a) Is there any problem due to cyst and not starting folic and multivitamin 3 months earlier ?
b) What may be the remedy of above two conditions , and reports ?
c)Now in future for a healthy baby what kind of precautions , eating habits , things should her do ?
d)Shall I stop sex with her ?
e)Shall her take other medicine too or should continue above treatment .
f) other recommendation .

I have seen all the attached reports.
a) The cyst can be physiological and may subside on its own in next 1-2 months. It is not likely to affect the baby. It is recommended to start folic acid 3 months before conception. But in case of unplanned pregnancy it is best to start taking it as soon as possible.
b) For nausea and headache, medicine Gudday will help. In case the nausea persists then she can increase the dosage to twice a day after consulting with her gynaecologist.
To increase the haemoglobin levels and other parameters, iron supplements will be started after 14 weeks.
c) She needs to have a healthy food, avoid junk food , have plenty of fruits and vegetables and protein rich food like milk and pulses.
d) She should avoid having sex till 3 months complete.
e) Do not take any other medicine except those recommended by your gynaecologist.
f) Do take proper rest and sleep, avoid spicy foods, go for regular check ups and keep me posted regarding the progress.

Thank you very much for guidance till now and i hope under your guidance we'll have a healthy mother and baby .

Please have a look on above discussion and reports and i have also uploaded latest reports of today's checkup . Detailed health conditions were discussed on first question .

my wife's last period was on 05 XXXXXXX 2021 and today on 13/03/21 again for the second time , i contacted doctor here in my town , as per her recommendation .

my doctor recommended sonography and after watching the reports of sonography , she said cyst is now gone and baby is growing .

I want to mainly discuss pain in my wife's abdomen area . main site of pain mainly just below the left rib . and also may be on variable locations . As my wife is having pain most of times , it maybe mild and a little bit intense also , but my wife said she can manage it anyhow . pain goes away for some time and comes back .

After discussing above problem with my doctor , she said baby is making space thats why pain is occurring , and pain is not a good signal , for that doctor recommended bottle and injection for 3 days and other medication for a month .


Details of medication recommended today are as follows :- ( plz let me know correction in dose and timing of medication , if any )

1. bottel - compound sodium lactate injection
injection - megapen 1g ( ampicillin and cloxacillin )
injection - aciloc 25 mg
injection - dromitin (drotaverine hydrochloride )
× all FOR 3 DAYS
2. tab - irotrop ( ferrus bis glycinate , zinc bis glycinate , folic acid , methylcobalmin ) ( everyday morning after lunch )
3. tab - raver it ( rabeprazole sodium , itopride hydrochloride ) ( daily morning empty stomach )
4. capsule - imufolra ( lactobacillus and others ) ( daily afternoon after lunch )
5. tab - yonocal d ( dicalcium malate , vit D3 , folic acid and glycine ) ( daily night after meal)
6. tab- drotamet 80mg ( drotaverine ) , when pain in abdomen , morning and evening After meal
7. LS PRO protein powder ( 2 teaspoon with milk , morning and evening )
8. syp - enzy ( digestive enzymes syrp with vit b complex ) ( 2 teaspoon After meal morning and evening )

Now i want to ask following questions :-
1. is really because of baby is making space thats why pain happens to my wife , if no then what may be the other reason ?
if yes , then bottle and injection will be helpful ? and if there will be any side effects of bottle and injection then ?
as i have discussed my wife's pain in details earlier above , what should my wife do , should her take bottle and injection or not .
after telling the pain site , sonographer not scan on site of pain that is below the left rib , why ?
2. shall my wife continue other tablets and medication recommended as per above details or any correction ?
3. on the updated ultrasound picture and report is it obvious that cyst had gone ?
4. when my wife's vaccination plan will start ?

1. The three days of injection treatment cannot assure that the pain will be gone. Without a physical examination it is difficult to find out the exact cause online. The treating doctor is the best judge in this case. You will have to have complete faith on her competence. The low levels of calcium during pregnancy do take time to recover.
2. In the second scan, there is no mention of the cyst. It means that the cyst is no longer present. During her next scan you can mention regarding the cyst before the scan and request the doctor conducting the scan to look once again for any cyst.
3. Low levels of calcium can cause pain in both muscles and bones. Lower rib cage pain can be present. This can be aggravated by gas and indigestion.
4. You can get a repeat scan done in case the pain is persistent or increases further. Or after one month to check the fetal growth.
