Question: Hi doctor .....

One month ago I have blood in my semen in two episode. I consulted with urologist and as per him I have done my urine and semen culture and with that my doctor prescribed me tab pause 500 mg 3 times in day for your day and tab zanocin 200 mg for two weeks. My both the culture report was negative.In between I felt little pain in my penis and burning sensation and the pain is in glain area of penis.As per my doctor advice I have taken inj mikasin 500 mg daily 2 times for five days along with tab niftas 100 mg for two week. But still I am having the penis pain and consulted with another urologist and as per advice I have done my urine routine, USG kub and PSA and all the reports are ok.Doctor has seen my penis ,testis and prostate and told everything is ok.I asked doctor if all the reports are ok then why is the pain and discomfort in my penis.He replied me just ignore the pain and apply some coconut oil and prescribed me tab uripas 2 Tab daily .

Now I am little bit worried about it.Please advice me what is to be done.My having constipation problem also and I do masturbate also.

Welcome to 'Ask A Doctor' service...

I've gone through your query and understand your concerns...

Well, it seems extensive work-ups been done already, and you've received the appropriate treatment only, so no need to worry first of all...

The fact that all these reports have come back okay is quite reassuring that there's nothing major as such. Please note, the penile area and other urogenital organs (Prostate notably) are highly vascular organs (rich in blood supply) and quite sensitive too (due to a lot of small nerve endings)...

There could've been some infective source/ organic inflammation/ frictions which may damage those nerve endings and cause such pain. Mild Prostate inflammation couldn't be visualized in USG-KUB (only high-resolution TRUS or MRI can detect to some extent)...

I'd concur with your Urologist, just continue taking Urispas, keep applying the coconut oil (do try to get extra virgin coconut oil for this purpose)...This may take several weeks to even months to fully resolve...Masturbation won't be harmful, but please be gentle for the time being, a brief period of abstinence may promote healing...

Regarding constipation, do not strain while passing motion...if the motion is hard, then you may take Syp. LACTULOSE 15ml once daily at bedtime and observe how you stay (the dose can be further escalated up to 30ml thrice daily based on the initial response)...

Drink plenty of water(at least 8-10cups per day), have plenty of fresh green vegetables, simple homemade foods, salads, curd based diet, avoid any form of spicy food...avoid smoking/alcohol strictly if at all do...

The pain/discomfort should reduce eventually with all these. However, if it persists beyond a few weeks, then you may discuss with your Urologist regarding medications like Amitriptyline (which is used to treat nerve pain in this scenario)...

Welcome back,

In an otherwise healthy young adult, there's actually no need to take Neurobion Forte...just well-balanced high protein diet, including plenty of fresh vegetables, salads, seasonal fruits should be enough...

This nerve issue is different and may not entirely respond to Neurobion forte. However, you can take it once daily after breakfast for the next 15days and observe how you stay...

Not to worry again Litan, just follow as already advised, you'd be fine...

Hi doctor....

Thank you for your valuable advice....I will follow your instruction.I need few instruction from your side.I have already taken 10 uripas tab and shall I continue it for few more days.And my pain started from 13th SEP and I am presently based at XXXXXXX and I consulted with urologist at XXXXXXX Now unable to contact with the doctor .In your first instruction u have mentioned some medicine named amitripline .As my pain and discomfort is about to complete a month .So can start that medicine which you told as my office work pressure is little bit high and some times bcoz of the pain I am unable to concentrate to my work also.Or it is required to consult with urologist before start such medicine.

Pls advice...

Welcome back,

Yes, as I've already explained earlier, just continue taking Urispas along with coconut oil application for the time being...

Please note, Amitriptyline is a strictly prescription based medication and should be taken only when under the supervision of your Urologist (as regular follow-ups required to note down the progress), so you should wait till further consultation with your Urologist...

Meanwhile, just follow all the advice given earlier, drink plenty of water throughout the day, have a balanced diet with plenty of seasonal fruits and vegetables. Perhaps practice a bit of yoga/meditation daily (about 30min would be fine), it'll be extremely helpful in the long run by reducing the daily stress...

Feel free to keep me posted regarding your progress afterwards...
