Question: Hi doctors. I have a very strange situation of breathing. I seem to breath hot air- very noticeable where somebody several fee away notices. Most often I notice how people around me cover their mouths and noses. It is very embarrassing as a teacher in front of class where all people up to 3rd row cover their noses, mouths and faces. Also breath bad smell when I go 3 hours without eating and have to keep eating and brushing to manage being around people (or other people around me). I have wondered if it is lung problem, ENT problem or what??? I am left clueless! Anybody with any ideas where to start treatment? Problem started maybe five years now?

Hii. This is may be dur to foul smell coming from your mouth or from your nose. This foul smell may be due to infection of the nose or the mouth. You need ton consult an Ent surgeon. There are many infection of nose like nasal polyps, allergic rhinitis, atrophic rhinitis. Atrophic rhinitis generally present with it. You need to get a nasal endoscopy done and get your nose and oral cavity checked. If needed you need to start a nasal fluticasone spray and saline spray to decrease the nasal discharge crusting so that smell can be reduced. It may be due to acid reflux. So drink more water. Start antacid. Keep on doing betadine gargle. Please review if whave any queries.

Thank you for your response Dr. XXXXXXX I am still quite confused. See the breathing hot air does not matter whether I use my mouth or nose... and if feels like the smell is also from stomach not from nose... I am assuming that is why the smell goes away when I eat a snack or food... however, right after eating is when the breath is heavier and more uncomfortable than when my teeth is freshly brushed... I don't know seem to have any other symptoms of acid reflux.... do you have any other additional thoughts? I also have a small swelling on my back- the surgeon I saw called it a lipoma and told me that it was totally harmless and to go for surgery only if it causes me uncomfort... I have also wondered if that small swelling could be the cause of this problem? Any ideas? thoughts on this?

Hii. Swelling in the back has nothing to do with smell. Get relaxed. Drink more water, Continue pantoprazole tablet. Sleep after two hours of dinner. You can some fibres in the diet to clear your bowel. In the mean time use some mint freshner to clear up ur breath. Nothing to worry. Do betadine gargle intermittently. Stay relaxed.
