Question: Hi dr Sheppe, I am at 3.75mg of Celexa, surprisingly it takes away my anxiety and OCD! But I am very wired, I cannot sleep without 1/4 of 50mg of Trazadone. I am extremely sensitive to medication, I became bipolar on 25mg of Celexa for a week and 17.5mg for 4 weeks. I became extremely depressed with suicidal thoughts and then, I was manic again. My questions is- will any antidepressant cause me anxiety/feeling wired at a low dose? My psychiatrist wants to put me on Latuda to get rid of my OCD, I refuse. I would love to take an antidepressant and be able to sleep. I was on paxil (half of the recommended dose) 3-4 years ago and wasn't wired at all. I was also on 2.5mg of Lexapro 2 years ago without any major issues. I took celexa 20mg 6 years for a year and a half and I had expired celexa (5 years expiration date) that I have been taking for over a year (i think I was taking 5 or 10mg) and I wasn't wired at all. My psychiatrist thinks any antidepressant will make me wired and anxious. Please let me know and thank you

From the available description it is almost clear that you are having hidden Bipolar disorder which needs to be managed properly. Since the presentation is often with depressive symptoms there is need to use a kind of medications which is useful for bipolar depression. The options include Latuda, Seroquel and Lamotrigine. These medications are the only options which will prevent wiring of your brain and treat the condition effectively.

I completely agree with your psychiatrist that any antidepressant will bring out the hidden mania and will bring more problems than help.

Thank you Doctor for your reply. I have never ever in my life experienced any symptoms of bipolar disorder!!! I have already had anxiety, and a slight OCD. I have never had mania or depression where I would not want to get out of bed etc, never ever in my life. Why do you think I have it now?... the pills made me develop it. Please explain

I understand that you never had any natural episode of mania or hypomania but for individuals with bipolar disorder type 2 it is natural to have predominant depressive symptoms and only on certain occasion symptoms of mania or hypomania could be elicited. While subjecting to antidepressant they have either unusual response or have manic/hypomanic symptoms as in your case. The available research clearly indicates that onset of manic/hypomanic symptoms indicates hidden bipolar disorder rather than true effect of antidepressants.

Considering this fact I feel that there is bipolar disorder which is underlying cause for your depressive and anxiety symptoms.
