Question: Hi, fairly urgent question here. My girlfriend (19) height 5,4, weight 54kg is getting a breast reduction surgery tomorrow. The problem is she found some rules that seemed to have changed that we’re concerned about. It said to have no tobacco products 6 weeks before the surgery and no alcohol 48 hours before the surgery. So she had one Snus packet in her mouth for a couple of minutes. (A tobacco product you put under your lip.) and that was two weeks ago. She had quit snus for over a year now so she has just had that one sample recently. She has also had some rum and coke yesterday at about 10pm she didn’t have too much. Maybe one or two of them. Definitely not enough to be drunk. I think she got some outdated rules for the surgery bc I think she thought it was originally, no tobacco products two weeks before and no alcohol the day before the surgery. This surgery is incredibly important to her and it would crush her if this was to stop it. But we also don’t want to put her in any danger at all. I am very worried about her. My personal opinion is she has a fragile body as well. She has chronic migraines and trouble breathing through nose recently and she gets sick very easily. That’s just my personal opinion though and why I’m also worried about her. So please tell me what we should do. Should we ignore it because it wasn’t that much or should we tell the surgeons about it ? Hope to hear from someone soon. Kind regards.

Thanks for your detailed history and query.
I can understand the concerns.
Both the things mentioned in rules is for the best outcome of anesthesia and surgery.
Since snus was 2 weeks ago there may not be much concern if she has not repeated it in last 2 weeks.
Small quantity of rum yesterday may not worry much.
Breast reduction surgery as you mentioned is incredibly important to her but this is not an emergency and if at all it is postponed should not really bother much.
But the decision takers are the Anesthetist and the Surgeon. Hence you should tell all this to both beforehand.
It is ultimately then to decide.

Thank you for your answer. My girlfriend is being very stubborn with not telling the surgeons about this. It is that important to her that in her head she can’t risk losing the surgery now. But I think I might have convinced her to tell them about the alcohol. However, i am having trouble convincing her to tell them about the snus. Is this something I should be satisfied with ? Should I leave it there. Or should I tell the surgeons myself that she has had snus ? I just don’t know how worried I should be. How much it would risk her health or life. I don’t want to go against my girlfriends wishes but if telling them this saves her from risking her health or her life then I think that’s worth doing. And I know the snus is way out of her system. She had it 12-13 days ago and she only has it for two minutes and she hasn’t had any since and she hasn’t had any tobacco products a year before that. So I know her system isn’t full of tobacco. So it is probably nothing to be worried about. But 12-13 days is also a big comparison to 6 weeks that are in the rules for tobacco products. I just want to know, if she doesn’t end up telling them about the snus, is this something I should tell them or tell someone about myself ? Since I am the only one else who knows about it. I’m just very worried about her and I want everything to be safe for her.

You must tell the truth to Doctors in private, you must.

Detailed Answer:
This is a great way to have a concern.
As we discussed in the beginning, it may not cause problems.
But being so cautious and worried about her you should inform the Doctors in private and let them decide what to do. This is the best way to care for her.
Let it be decision of the Doctors - not hers, not yours, not mine as they know the vitals and other aspects of the patient - your friend.
I am sure the Doctor may not divulge anything with her if you tell the truth in person and in confidence. And this is really needed.
Doctors too have their ethics so do not worry. Tell the truth.
