Hello and welcome to "Ask a Doctor" service.
I have read your query and here is my advice.
In my opinion, this is not a case of MND (motor neuron disease).
Factors against MND in your boyfriend's case include: onset of symptoms (two years is a long period for MND without other symtoms or worsening), only twitching, no other symptoms ( no muscles atrophy, weakness).
Medical examination and tests also that didn't support MND are other factors to consider.
So, in conclusion, I think MND is a very unlikely condition in your boyfriend's case.
Hope you found the answer helpful.
Let me know if I can assist you further.
