The possible causes of the shooting pain are the healing tissues, instrumentation, involuting uterus, ensuing menstrual cycle and infection. If the discharge is foul smelling and associated with itching/spotting etc., infection should be suspected. Simple excess vaginal discharge can occur at the time of ovulation and also just before the onset of a period.
Any forgotten tampons should be excluded. A clinical checkup should be conducted by her consultant to assess the post-abortal state. Anti-spasmodic medication helps in relieving the spasmodic pain. Swabs may be ordered from the cervix and vagina to exclude infection. A sonogram of the pelvis, preferably in the trans-vaginal route, helps in further assessment of the situation.
Ovulation can recur within a month of the abortion, especially if it happened during the first or early second trimester. If she is sexually active without any protection, pregnancy also should be suspected.
In view of all these, I would ask her to see her consultant for further management.

Thankyou for your response. I want to also add a couple points below regarding her symtpoms to see if possibly you can tell what is wrong.
- when she is sitting down the sharp pains are the worst
- Her stomach gets bloated
- the vaginal discharge which she says is just like water no colour or odour, only started this week.
hope this can make her symtpoms a little clearer.

The possible reason for the increase in the pain while sitting is rise in the pelvic pressure. The associated bloating is likely to be a gaseous distension or increase in the pelvic volume.
I would still ask you to get a clinical evaluation and a sonogram to clarify the situation.
I would like to exclude any pelvic infection, pregnancy, bowel or urinary causes etc.
